How Does It Work?
• One week before the patient's scheduled pick-up or
delivery, the pharmacy will call the patient.
• The pharmacy reviews the patient's medications
with them, to evaluate adherence and ADRs, answer
any questions, and look for possible ways to save the
patient money.
• The pharmacist will review the order and contact the
prescriber's office if refills are needed or the patient
reports a change in therapy.
• The patient will receive a reminder call when the
refills are ready.
How Do Patients Enroll?
1. Patients can self-enroll at the
pharmacy or can be referred by the
prescriber.
2. They bring all of their prescriptions
as well as their vitamins or
supplements to the appointment.
3. Patient and pharmacy establishes refill date.
4. Patient and the physician receive a summary of the
patient's drug list including medications and
prescribers.
