 Discuss scope of rated officer’s duties within

period utilizing the DA Form 67-9-1
 Review DA Form 67-9-1 at end of rating
period
 Verify and enter the rated officer’s APFT and
height and weight data on DA Form 67-9
 Assess and evaluate performance of officer
on DA Form 67-9

 Minimum of 60 days
 Assess performance of the rated officer
through DA Form 67-9
 If appropriate, provide information on DA
Form 67-9-1 to assist senior rater

RESPONSIBILITIES
 60 day minimum
 Ensures support form is provided
 Become familiar with rated officer’s
performance
 Assess rated officer, considering DA Form
67-9-1 and potential relative to his/her
contemporaries
 Has rated officer sign the DA Form 67-9
