ADA Living Well With Diabetes interactive, print on-demand,and booklets The American Diabetes Association in collaboration with StayWell is providing this complete suite of self-management and curriculum resources at no charge to all ADA-Recognized Program staff. These resources include web-based tools, print, and interactive Go-to-Guides™ you can use with your patients. These patient resources were created by Krames Patient Education, a product of StayWell in collaboration with the American Diabetes Association and sponsored by Merck via their Journey for Control Program. Staywell worked with health care providers, researchers and diabetes educators to select the most appropriate and helpful information for diabetes self-management, presented in reader-friendly format. Arranged in folders for easy, print on-demand access to support your busy practice these tools are accessible from any computer, tablet, or smart-device with internet access. The information is consistent with all ADA-approved educational content. The next few slides will guide you through the resources and provide links to additional support as you incorporate these innovative self-help tools into your practice. All rights reserved. The following examples are from ADA Recognized Program Staff illustrate how the Living Well with Diabetes workbook is being used in daily practice. IN A GROUP SESSION… At the Nebraska Medical Center Diabetes Center, we currently use the Krames Living Well with Diabetes self-care workbook as the principle written reference for our Type 2 diabetes group class. The chapters in the workbook follow our weekly class outlines very nicely! Our Type 2 group class is divided into four weekly sessions. Our weekly topics are: Week Week Week Week 1: 2: 3: 4: Understanding diabetes, use of meters and monitoring and medications Creating a healthy meal plan Being active Maintaining overall health including complication prevention and problem solving blood sugar problems The information in "Living well with Diabetes" is clear and concise and the photos show a wonderful variety of people who appear to be enjoying their life while they are managing their diabetes. Patient and staff alike are very pleased with how this Krames handout is enhancing our classes and we thank you for it! For our Annual Visits we use the Conversation Maps and Living With Diabetes booklets. Our patients enjoy the visual cues from both tools to help in discussions on topics of specific interest to them. There’s so much we COULD cover, but need to keep these Annual Visit sessions to a two hour segment, so these tools help us prioritize the interests of the group gathered on any given day. Patients often tell us at the end that they really do like the format of the Conversation Maps and appreciate having the Living Well booklets as a new, current resource to take home with them at those sessions. This booklet is an excellent overview to get patients started on healthy habits while preparing for their education classes. Patients have provided feedback on booklets, enjoyed it as a resource. We check pre and post Hgb A1C, weight, and blood pressure values for our program, with over 95% of patients showing Hgb A1C reductions. Thanks for the resources! I use the Go-to-Guide™ in my classes and it is easy for the patients to follow.” INTEGRATING ALL THREE The Perspectives Diabetes Education Program offers a packet of valuable resources during our first group class. The attached picture shows that two of our favorite handouts are the Living Well with Diabetes book and “Bookmark It” flyer for the Diabetes to Go Guide™. The Living Well with Diabetes book reinforces the teaching that we provide, especially during our first group class that discusses “what is diabetes,” living healthy with diabetes (avoiding chronic complications), medications, monitoring, and healthy coping/behavioral change. EVERY participant enrolled in our program receives this book. For our participants with diabetes that have internet access, the interactive Diabetes To Go Guide is fun because they can listen all over again to things we have said about good diabetes self-care. We also use Krames On-Demand handouts, especially the Prediabetes and nutrition handouts, to send or give to people who want a little more information, but who do not qualify for our program. REACHING DIVERSE POPULATIONS We use the Living Well with Diabetes Workbook as the initial print material given to each nongestational diabetes client enrolled in the ADA Outpatient DSMT program at Texas Health Harris Methodist Fort Worth. We use the Living Well with Diabetes Workbook as the initial print material given to each nongestational Diabetes client enrolled in our second site ADA Outpatient DSMT program at Texas Health Harris Methodist Southwest. The Living Well with Diabetes Workbook serves as the foundation material for a free Diabetes program designed specifically to meet the needs of the unfunded / low income clients in our service area who are unable to attend our ADA Recognized programs because of financial barriers. We used the Spanish Living Well with Diabetes Workbook as the support material when we took our program into the community, conducting free diabetes classes in Spanish for a non-English speaking congregation in a low income area over a series of four Sundays after church service. Participants loved the material! We used the Living Well with Diabetes workbooks as support material when we presented a free Diabetes Update program to a group of patients enrolled in an outpatient Cardiac Rehab program in our community. Great material. It allows space for us to write in notes individualized to the client, and for them to write notes as we cover the material. Thanks for making this resource available to us!
