Service Design 2015-2016 April 13, 2015 Goals Review the design of the Ward, ICU, and ambulatory rotations This is NOT schedule picks April 17th for rising 3rd years April 20th for rising 2nd years General Schedule Interns: 7-9 Ward/ICU months PGY2: 8 Ward/ICU months, 3 clinic months, 1 consult PGY3: 6 Ward/ICU months, 3 clinic months, 2-3 consult months Surge 2nd years only have 1 consult month available for surge so will do 7-8 days consecutive 3rd years carry the majority of surge Gen Med Overview 4 teams at the Main Two 1 resident, 2 intern teams Two 1 resident, 3 intern teams 1 team at ART 1 resident, 2 interns 3 teams at the VA Two 1 resident, 2 interns One 2 resident (rotating nights), 2 interns Gen Med Service: Main 4 Teams Team 1 and 3 1 upper level and 2 interns Team 2 and 4 1 upper level and 3 interns because of night float Call Residents: q4days Weekdays: 8AM-8PM Fri/Sat: 24+4hr call Sunday: 8AM-5PM Interns: q4days All days of week: 8AM-8PM Gen Med Service: ART 1 upper level, 2 interns Call Resident: q3days Weekdays: 8AM-8PM * Fri/Sat: 24+4hr call Sunday: 8AM-5PM Intern: q4days All days of week: 8AM-8PM Cross Coverage and night admissions will be for Liver/Heme and Gen Med services *Day float help 4-8pm VA Wards 3 Teams: Red, White, Blue Red, Blue 1 upper level 2 interns White 2 upper levels (2weeks days, 2weeks night) 2 interns Day float- 1 resident alternating 8-5pm, 8-8pm Mon-Fri Call Intern: Intern NF of 3 and 2 days; day calls shared by 5 interns not on nights Resident: Weekdays: q3days but alternate weeks, 8-5PM and 8-8PM, with day float resident Weekends (Fri/Sat): 24+4hr calls Sun: 8AM-5PM MICU 5 residents and ~3 interns ER residents will continue to be in the mix as upper levels Call Intern: ~6calls throughout the month with 6 nights of night float Resident: q4days, 8AM-8PM Resident Night float, 8PM-8AM 6 nights for each resident VICU 3 Residents Call: 24+4hr shifts q3days Cardiology Wards 2 Teams 1 upper level and 2 interns +/- midlevel involvement Day float 4PM-MN, Sun-Thurs Night float 8PM-8AM, Sun-Thurs Call Resident Weekdays: 8AM-5PM Friday: 5PM-8AM covered by UIM clinic resident Saturday: 24+4hr call Intern: q4days Weekdays: 8AM-8PM Fri/Sat: rotating day or night GI Liver 1 upper level, 1 intern Call Resident: q3days Weekdays: 8AM-8PM Fri/Sat: 24+4hr call Sunday: 8AM-5PM Intern: q4days Weekdays: 8AM-8PM Fri/Sat: rotating day or night call Heme Wards 1 upper level, 1 intern Call Resident: q3days Weekdays: 8AM-8PM Fri/Sat: 24+4hr call Sunday: 8AM-5PM Intern: q4days Weekdays: 8AM-8PM Fri/Sat: rotating day or night call Night float 4 Residents Gen Med Sun 5PM-8AM Mon-Thurs: 8PM-8AM ARTGM/Liver/Heme Sun 5PM-8AM Mon-Thurs: 8PM-8AM Cardiology Day Float Sun-Thurs: 4-12pm Cardiology Night Float Sun 8PM-8AM Mon-Thurs: 8PM-8AM GEM/Day float 2-3 Residents 1 resident always on Liver/Heme/ART GM Day float (4pm – midnight), Monday-Thursday 1-2 resident(s) on GEM Weekends to be divided between the 2-3 residents VA Day Float 1 resident per month Week 1: Mon-Fri 8AM-5PM Week 2: Mon-Fri 8AM-8PM Etc, Alternating weekly shifts with the ward residents UIM Clinic Upper Levels: UIM Clinic q4months 6 continuity clinic ½ days Admin ½ day Other ½ days are subspecialty Interns: Weekly continuity clinic PLUS 1 Intern UIM Ambulatory month Total of 3 ½ days of continuity clinic PLUS reading EKGs, PFTs, Stress tests, etc and other subspecialty clinics Ambulatory Experience Mon AM: Rismiller (RT) or Argula (RT) Mon PM: Chhabra (HCC) or Van Bakel (ART) Tues AM: Brock (ART) or Sherman or Usher (ART) Tues PM: Castell (ART) or O’Brien (ART) Wed AM: Zwerner (ART) or Ngo (RT) Wed PM: Huggins (RT) Thurs AM: Christiansen (HCC) or Morris (ART) Thurs PM: Boger/Gnann (RT) or Van Bakel (ART) Fri AM: RAC Fri PM: RAC *Subject to change Schedule Preference Day Rules #1- Rising PGY3s in JULY have to be in UIM Clinic OR Ward/ICU month…no consults #2- You will make at least 3 picks. #3- One pick must be ICU #4- One pick must be wards or night float #5- Know your picks ahead of time. Time is limited Remember these are only preferences. We cannot guarantee them. Make picks in order of importance to you. PGY3s You need: 6 Ward/ICU 2 ICU 1 night float 1 GEM 2 other 3 UIM Clinic 3 consults/ambulatory month PGY2s You need: 8 Ward/ICU 1 night float 1 VICU 1 MICU 5 other: Gen Med, VA wards, ART GM, Liver, Heme, Cards, Day Float 3 UIM Clinic 1 consult Graduation Requirements 3 AMBULATORY MONTHS outside UIM Clinic (ideally 1 PGY2 and rest PGY3 yr) 1 ER month (VAUC or Main ED) Neuro: Stroke service as intern or Consults as upper level Geriatrics (GEM) Questions?
