Patient Discharge Letter (1 copy) given to pt • Patient Instruction Sheet (1 copy)

Nursing no longer needs to complete the handwritten Discharge Instruction Sheet.

– shorter version of the Patient Discharge Letter – Nurse uses to review discharge instructions with the patient – Requires signatures of nurse and patient – Is placed in chart as a permanent document • Asthma Action Plan (2 copies) – one copy is given to patient – Other copy is signed by nurse and patient and placed in chart as permanent document • Prescriptions: are still handwritten
