





Referral - Court, Psychiatric Colleagues, Care

Assessment - background information, need for
treatment, learning ability or disability
Rolling programme - join at any time dependent on
dynamics
Individualised intervention within group therapy
Re assessment at six monthly intervals to assess
progress and effectiveness
No time set for intervention but on average adapted
three year programme

Discharge pathway








When ready - three months prior to discharge
discussion with client – do you feel ready?
Planned discharge
Discussion and planning with care provider
Risk management
Plan to replace time at group therapy with other
activity
Address any staff anxiety re discharge
Ongoing open case for individual follow up over a
twelve month period - where possible
Continued attendance at Risk Management/MAPPA
meetings
