NEXTGEN TOBACCO STATUS DEMONSTRATION This demonstration reviews documentation of tobacco status in NextGen. This has been prepared with EHR 5.6.5.63 and KBM 7.9.2. Subsequent updates may display cosmetic and functional changes. Use the keyboard or mouse to pause, review, and resume as necessary. There is no audio with this exercise. Tobacco vs Smoking History As of this writing (summer 2011), documenting tobacco status in NextGen has become more complicated and less intuitive. This is largely due to the fact that Meaningful Use specifies the recording of smoking status, whereas good medical practice documents the abuse of any form of tobacco. As a result, tobacco documentation has become somewhat redundant. On the Histories Tab, note there are now references to both smoking and tobacco status. To begin documentation of this, click Tobacco Usage. For someone who has never used tobacco, click never, then Add. This status is added to the Tobacco Use Grids. For someone who currently uses tobacco, click current. Add other details as known/pertinent, then click Add. Here we’ve indicated that the patient is a current cigarette smoker. One might logically think this would be sufficient, and you could now click Save and Close. However, if you do, you will see this popup, regardless of the tobacco status you’ve indicated. That’s because we’re now expected to also indicate smoking status as well before clicking Save and Close. Yes, illogically, even if we’ve indicated that the patient has never used tobacco, we have to also say that the patient has never smoked. After clicking Save and Close you’ll return to the Histories Tab and see the data that has been entered. Another variant you’ll see occurs when a patient age 13-18 is seen. The pediatric version of the Histories Tab doesn’t allow you to directly enter the patient’s tobacco history, yet Meaningful Use requires that smoking status be documented. As a result, we’re seeing these overlapping fields, including the Required for MU notice. (It is unclear whether NextGen considers this a “feature” or an error.) One way to enter tobacco history for an adolescent is to click the Social History Update button. (This is also the way you’d enter other Social History details on children.) The Pediatric Social History template appears. Change to the Adult Social History template by clicking Adult Social History. From here you can document the patient’s personal tobacco history by clicking the Tobacco Usage link as demonstrated above. Here is another tip: Note the red text Required for MU. Almost everywhere in NextGen red text is a warning or notice, and blue text is an active link. However, here Required for MU is also an active link. For both children and adults, clicking this link brings up the Tobacco Use popup, allowing documentation of tobacco/smoking status. Thus this gives us a direct way to document this on patients 18 and under that saves a couple clicks. Obviously, this workflow presents some annoyances and frustrations to the user.
