• Hospital code:
o Allocated by HPSC to each PPS team leader
• Start and end date for the survey dates in your
hospital
o End date is the date the data was collected on the last
ward
o DD/MM/YYYY
• Hospital size (total number of beds)
o Total number of in patient beds in the hospital (include
designated long term care ward) psychiatric
• Number of acute care beds
o If no designated long-term ward; the number of acute
beds = total number of beds
• Number of ICU beds
o Number of intensive care unit beds – if no ICU then = 0
• Exclusion of wards for PPS?
o Were any wards excluded for the PPS Yes or No?
• Is yes –
o Use ward speciality list to describe (Appendix a Table 1)
• Total number of beds in included wards
o Sum up the data from all the ward lists to derive this
number
• Total number of patients included in the PPS
o Use all the ward lists to calculate this number
• Primary
• Usually called a general hospital
• Some specialties (general medicine, general surgical, obstetrics,
paediatrics)
• Secondary
• Usually called a regional hospital
• 5-10 clinical specialties e.g., oncology, haematology, nephrology, ICU
• Takes referrals from general hospitals
• Tertiary
• Hospital with national specialities
• Specialist staff e.g. haematology, ICU, cardiac- thoracic, transplantation
• Takes referrals from primary and secondary hospitals
• Specialist
• Single clinical speciality e.g. maternity, orthopaedic, paediatric
• Number of admissions and patients days
• Source data for 2011 from administration staff
• Alcohol hand rub consumption
• Preferably for 2011, but if not state year
• Year - ‘11’ for 2011, ‘12’ for 2012
• Use ward lists to calculate the:
– No. of patient rooms (add the number of single and multibedded rooms together)
– No. of single rooms
• No. of FTE of IPCNs and infection control doctors in
your hospital
– Infection control doctor = doctor with a specific infection
control role designated in his/her contract of employment
• This data applies to 2012 – ‘12’
• Finally
– Input the data from this form into the web-based
system
