Daily logs and
 Medical Billing

 This
year a change is
coming!
Daily Documentation

For student receiving Special
Education services the Services
Tracker Medicaid Logs will be
used as the required logs for our
case record.
They are to be completed and
printed out monthly to put in our
case record.
 Do this before you bill.

Daily Documentation

For daily documentation for
Regular Education student, we
will continue to use our daily
logs forms that we implemented
last school year.

This form can be found on the
DPS intranet in both a electronic
form and a printable form.

When completing your daily logs
the following must be included:
Service logs must describe the
focus of the service provided
(which should correspond with the
student’s current treatment goals).
 They must also note the student’s
response to intervention.


Examples


Student focused on self control.
Participated in conflict resolution role
play. Brainstorm positive alternatives.
Student’s was engaged and his input
was insightful.
Student worked on relaxation
techniques to assist with anger
management . His peer interaction
showed improvement. He was
engaged, on task and receptive.

The monthly progress note
(required for special education
documentation only) must:

Briefly summarize service
provided, progress made that
month and plan for next month.

Example

Met with student twice this month,
focused on improving self control.
Student is receptive to the need
for change. Work on this goal will
continue.

(Lowercase first name initial,
Uppercase last name initial, day of
birth-month of birth)


You can submit billing for a
student over your monthly IEP
hours

They will average it out. Example,
you have a student who is seen
three times monthly. In January
you see him once and in February
you see him 5 times. This totaled
6 which will make up for the time
missed in January.

Note all of your Unbillable services
under this heading for the entire day.
You can only use this heading once
a day.
You can bill for Crisis any number of
times. The student does not have to
be on your caselaod and does not
have to have Social Work Service on
their IEP.

How to Print monthly service report:






After logging in go to Zangle report.
Then click on service tracker and follow
the link to service report.
Next click on monthly service report
Put in you school name, month and year
of service.
Click create report
