COMMUNITY PARTNERS PROGRAMME CPP Federal Government Programme FOR ELDERLY PEOPLE FROM ETHNIC COMMUNITIES THE COMMUNITY PARTNERS PROGRAMME (CPP) As we get older we may choose to get support in the home or in the community. Later we may want to live in an aged care facility. Organisations that provide support to the elderly are called “aged care support service providers”. WHAT DOES THE CPP WORKER DO? The CPP worker is the link between the aged care services and the Hungarian community We tell aged care services about the Hungarian community And we tell the Hungarian community about aged care services Aged Care Service Providers CPP Nelke and Julia Hungarian community WHY? SO THAT ELDERLY HUNGARIAN PEOPLE HAVE GREATER ACCESS TO CULTURALLY APPROPRIATE AGED CARE SUPPORT SERVICES HOW: The CPP worker gives information to the Hungarian community on aged care services so you can make good decisions AND The CPP worker teaches Aged care services about the Hungarian community and their needs so they understand you better WHAT IS CULTURALLY APPROPRIATE CARE? Culturally appropriate care aims to provide what is important to you! Culturally appropriate meals Lecsó WHAT IS CULTURALLY APPROPRIATE CARE? Recognises Hungarian cultural celebrations and supports activities and festivities throughout the year WHAT IS CULTURALLY APPROPRIATE CARE? Celebrates Religious days Kouglof Beigli WHAT IS CULTURALLY APPROPRIATE CARE? - Welcomes the local Hungarian community and families visiting residents - Helps display cultural artifacts - Provides bi lingual or bi cultural workers - Provides literature in Hungarian - Uses interpreters More aged care support services delivering culturally appropriate care to older Hungarian people HOW? CPP workers to: - Meet with Aged Care Support Services in the Illawarra - Provide Hungarian cultural information kits - Teach Aged Care Services on Hungarian culture and the needs of elderly Hungarian people CULTURAL LESSONS The CPP will teach aged care support service providers about your community - Culture - Traditions - Family life - History CULTURAL LESSONS - Migration story - Settlement in Australia - Life in the Illawarra INFORMATION MEETINGS Teach elderly Hungarians about aged care support services so you are able to choose what you want. THE CPP PROJECT SUPPORT IN THE HOME: INFORMATION MEETINGS Teach elderly Hungarians about aged care support services so you are able to choose what you want. SUPPORT IN THE COMMUNITY: INFORMATION MEETINGS - Once every quarter - Guest speaker from Aged Care Service provider will visit - Interpreter - Information in your language and English HOW YOU WILL BE ABLE TO MAKE THE RIGHT DECISION - You will learn about the types of aged care support services available - How much they cost - Your rights HOW WILL YOU KNOW ABOUT THESE INFORMATION MEETINGS Advertising through a two page newsletter Telling each other about it We would like your family members at the meetings INFORMATION SESSIONS THANK YOU Julia Wren and Nelke Willow Community Partners Programme CPP
