“LET’S LOOK AT THE NEW
REPORT.”
Look at the Monthly Report
you printed.
– Your school goes on the
top line.
– The second row lists the
months.
– The left-hand column
lists 14 headings.
14 Headings
These 14
headings capture
the data…
ADE wants us to
collect.
Monthly Report
This form reports
the total number of
hours each month.
Look at the yellow
row with the
months.
The 14 headings
are on the left.
Read the purple
headings.
WHAT?
• How can I make
sense of all these
boxes?

Report—Month
• Week
On this
form you
can keep
track of the
hours you
spend
weekly.
You can
make a
paper copy
of the form
or download
the Excel
file from this
site.
Fill in the
yellow
boxes.
For “WEEK” use the
weeks for the month.
For example, if you are
tracking hours in
September, the weeks
would be:
o 1-5
o 8-12
o 15-19
o 22-26
BUT, what
about the
extra days in
September?
Put them where
you like. This form
is for your use
only.
At the end of the month, total your
hours and transfer them to the
Monthly Report.
