Revised Death Notification (F5) been in use since
December 2013
Current process is reliant on transcribing telephone
calls
• Creates delays in reporting for both NHS and
COPFS staff
• Not a good use of consultant or clinical time!
Similar process as eMCCD to be used for sending
information to COPFS

Piloted with Hospices from October 2014

•
Mesothelioma pro-forma:
• Family should hold documentation in partnership with
GP
• SGPC suggest scan and attach to eF5
• SG contacted that process is not universally
understood or followed
•
Requests for additional information:
• Will be accompanied by telephone alert (as for
outcomes)
• Case number and UCPN will be required for
response (copy & paste)
MCCD - What has changed?

Approx 55,000 deaths in Scotland per year
Previous related legislation for deaths, burials and
cremations was over 100 years old
SG review (Sheriff Brodie) led to refreshed legislation
Certification of Death (Scotland) Act 2011 introduces:
• Revised MCCD
• New system of scrutiny for MCCDs
• Electronic version of MCCD
Revised MCCD was introduced in August 2014
Launch of new measures – 13 May 2015

Implementation to Primary Care and Hospices:
13 May 2015

Implementation Leads Group established with
representatives from each Health Board
• Actively progressing central issues register
•
SCI Gateway development included in content for R19
• Print module add-on required to conform to legal
document (MCCD)

Testing schedule:
• Internal testing (w/c 09/02)
• Component testing (w/c 23/02)
• Live environment end-to-end (April)

suitable communications route to ensure that the GP

eMCCD - Points to note
•
Printing:
• Failure of first print will result in the MCCD being
marked with ‘Duplicate’. This matches manual
process and is supported by NRS cross-check
activities
• Tests in NHS Fife suggest paper quality to be 80gsm
•
eMCCD Training activity – should be flagged in LIG
education plans
SCI Gateway - Points to note
•
Locum access – SCIMP / SGPC suggest locums use
manual MCCD pads where:
• Local practices support locums completing death
paperwork
• Access to SCI Gateway has not been achieved in
time
•
Printing:
• New module introduced in R19 to enable production
of PDF output for eMCCD
• Does not need client installed on PCs
• Does need an Adobe reader – understood to be in
standard desktop build
