all about the
Opportunity PassportTM
Participant Survey.

What is the Opportunity PassportTM
Participant Survey?
It’s a survey used to collect information
from young people enrolled in the
Opportunity PassportTM.
Sometimes it’s called OPPS for short.
OPPS is designed especially for young people.
Young people even helped create it!
Why should I take OPPS?
We want to keep learning as much
as possible about the lives of young
people leaving foster care…
By taking the survey, you are helping
people understand what life is like
for these young people.
For example, the survey asks things like…
Whether you
have a doctor
Where you
live
Whether you
have a job
Who supports
you most
How you pay
for housing
Why is all this information important?
The information can be used to improve
the system and create opportunities for
young people leaving foster care.
For example, one site’s survey results showed that young
people needed some help finishing high school...
So they hired an educational planner!
You can see that taking the survey is really important.

Your time is important too.
You’ll get paid $40 each time you
complete the survey !

And there’s more…
$

The $40 will be divided equally
between your IDA and debit
accounts ($20 in each account).
And, the $20 in your IDA
account is matched.
So, you actually earn a total of
$60
When do I take the survey?
The first survey should be taken
as soon as possible after you enroll
in the Opportunity PassportTM.
After that, you take it
every April and October.
But why do I need to take the
survey twice a year?
Because it helps us see how things
are changing over time.
How do I take the survey?
It’s online!
You will be given a user ID when you first
enroll in the Opportunity PassportTM.
You need it to take the survey.
I don’t have a computer
with Internet access.
How do I find one?
You could go to
a public library
…or a drop-in center
…or just ask the person helping you
with the Opportunity PassportTM.
How do I get to the survey
on the Internet?
You can access the survey through
either one of these websites:
www.transition.fosterclub.com
OR
www.jimcaseyyouth.org
No matter which website you use,
the survey will be exactly the same.

To take the survey,
click on the link
that says:
“Take the
Opportunity
Passport™
Participant
Survey here”

After clicking the survey link, you will see
this page.
The first time you take the survey,
enter your user ID.
You don’t need to enter a password yet.
Then press LOGIN.
On the next screen, enter your
email address.
Each time you take another survey, you
will be asked to confirm your current
email address.
Next, below your email address, create and
enter a password.
Pick a password that’s easy to remember, like
your birthday or a pet’s name.
If you ever forget your password, it can
be emailed to you.
…And then you can start taking the survey!
Can I take the survey in Spanish?
YES!
You may choose to take the survey in Spanish.
The format and all of the questions are the
same in the English and Spanish versions.
Here’s how you get to the Spanish survey:
When you first get to the login page, click the
link below the login button.
This will bring you to a Spanish login page, where
you can login and take the survey in Spanish!
