Care Plan Forms


Students are NOT allowed to carry medication on them to
school unless proper documentation has been filled out
with the school nurse.
If your student is diagnosed with seizures, bee stings/food
allergies, diabetes, or asthma, there are care plan forms in
the clinic. Please see the school nurse.
Special Medical Considerations

If your student needs to carry a water bottle or have
unlimited restroom privileges, the clinic needs to have the
doctor’s orders on file for those special considerations.
Lockers





Students will be assigned their own locker.
Students may not share lockers.
Combinations are confidential.
Lockers are a privilege- treat them with care.
Combination locks – start practicing !
Transportation



Students ride the bus with elementary students in the
morning. After school, all students ride a transfer bus to
the middle school. If the transfer bus is not their home
bus, they will switch at the middle school.
All changes in bus transportations will need to go through
the transportation department which they will then notify
the CBIS office to create a bus pass for the student.
