REGISTRATION
Get the information you’ll need to register by picking the class of your
choice. Make sure your schedule allows you to be present at the entire
class as listed under Class Times.

Make sure this page is the clinic you want to attend.
Click here to create your USSF profile:
Create a profile. Fields marked with * are mandatory fields. Arizona
does NOT require a SSN. Create your own User Id. The system will let
you know if your ID is not unique (note: email address is not allowed).
Create your own password and re-enter it under Confirm Password:
Verify your profile information is correct.
Then save
Your registration is Confirmed
Now Login Here
Enter your User Id & Password you created HERE
Then press Login
Choose Click here to Register to continue
Registering for your class
Your Information will be listed.
Check Referee to register as a Referee and to
have the list of classes appear
•Make sure you check the box for Referee
•Pick a class selecting the radio button next to the Clinic Name.
•If you are unsure you can click on To View Description
You must:
• read and answer the Conviction Question
• agree to the terms and conditions
• Enter Your Credit Card Number (Visa or MasterCard)
• If you do not have one, you may acquire a Visa Gift Card from any retailer that sells gift
cards.
• Make sure you have enough on the Gift card to cover your fees. Click continue.
• You then come to a Receipt page and a receipt will be emailed to
your email address. PRINT YOUR RECEIPT!
• You must bring a copy of your Receipt to the clinic to verify your
payment.
