Secure Materials
• Test booklets and answer
documents are secure materials.
• Both must be counted and signed
for each time they are checked out
and in.
Preparations in Advance
• Principal and building level coordinator must
determine how students are going to be grouped
for testing. Recommendations: Group students
together who typically take more time; pull out
students who are typically disruptive and test them
in smaller groups.
• Review the WVS.326 test accommodations report
for accuracy. It should match the IEPs. Contact the
case manager if changes are needed.
Preparing Students for Testing
• Science and math teachers should
provide practice for students using the
reference sheets.
• Students are allowed to have copies of
the reference sheets now. New copies
will be provided during testing.
– Periodic Table of the Elements (Grades 5 – 11)
– Science Reference Sheets (Grades 7 – 11)
– Mathematics Formula Sheets (Grades 9 – 11)
Preparing Students for Testing
• Ensure all students are familiar with both the
general types of questions on the test and the
procedures to follow when recording the answers.
– Third grade students mark their answers in the test
booklets.
– Fourth through eleventh grade students mark their
answers on a separate bubble sheet.
• Teach students in grades 3 – 11 how to mark the
gridded response answers for math; grades 6 – 11
must change mixed numbers to improper fractions
or decimals.

During Testing
• The principal/building level coordinator will monitor
testing rooms to ensure that proper testing
procedures are followed.
• Test booklets and answer documents will be checked
out each day and checked back in immediately after
daily testing. The School Security Checklist will be
used to log the transfer of testing materials from one
person to another.

Damaged Test Materials
Examiners must inform the principal/building level
coordinator immediately if a book or answer
document is damaged. The Principal/Building Level
Coordinator will obtain non-damaged material from
school overage or from county test coordinator
• Student responses must be transcribed to a nondamaged test booklet or answer sheet.
• The scribe must complete a WESTEST 2 Scribe
Verification Form.
• The principal/building level coordinator will complete
the student information on the Bio-Grid in the new
test book (grade 3) or answer document (grades 4 –
11).
Damaged/Contaminated Test
Booklets/Answer Documents
• If test materials are contaminated, follow
safety precautions when handling them (i.e.
rubber gloves, mask).
• Seal contaminated booklets and/or answer
documents in a plastic container/bag after
transcribing.
• The damaged or contaminated materials must
be returned to the county test coordinator.
• Never destroy or throw away any test
booklets or answer sheets.
Scenario
Mary attends public school and her teacher,
Mrs. Brown, is also her mother. Mrs. Brown
administered the test to her class.
3.16. Examiner. An examiner must be a currently employed
educator and/or an approved employee of the state, county
or RESA, or a currently employed educator of a nonpublic
school who holds a valid West Virginia teaching license or
certification granted by the Office of Professional Preparation.
The examiner may also include substitute teachers or aides
serving as an examiner for special needs students when they
are instructionally assigned. All aides and nonpublic school
educators must be approved by WVDE. Student teachers may
not serve as examiners. The examiner must have been
trained and must have on file a signed Examiner’s/Scribe’s
Secure Materials and Test Procedures Agreement for the
purpose of administering or assisting with the administration
of an assessment included in the WV-MAP. Examiners may
not administer WV-MAP assessments to his/her child.
Scenario
On the morning of testing, the Grade 3
Examiner has a medical emergency and
cannot be at school. The principal decided to
have an experienced kindergarten aide
administer the test to this class.
3.16. Examiner. An examiner must be a currently employed educator and/or an
approved employee of the state, county or RESA, or a currently employed
educator of a nonpublic school who holds a valid West Virginia teaching license
or certification granted by the Office of Professional Preparation. The examiner
may also include substitute teachers or aides serving as an examiner for special
needs students when they are instructionally assigned. All aides and nonpublic
school educators must be approved by WVDE. Student teachers may not serve
as examiners. The examiner must have been trained and must have on file a
signed Examiner’s/Scribe’s Secure Materials and Test Procedures Agreement for
the purpose of administering or assisting with the administration of an

3.4. Aide. For the purpose of testing, an aide is a person who meets or exceeds
the qualifications of Aide II pursuant to W.Va. Code §18A-4-8 (i)(9) and is hired
to work with special needs students in an instructional setting. These aides
may serve as an examiner for the special needs students to whom they are
instructionally assigned. All aides must be pre-approved by WVDE.

to questions is permissible in all content areas.
• A scribe may be used when a short-term
medical condition (e.g., a fractured arm in a
cast) precludes the student from writing with
the dominant hand to mark responses.
• Graph paper, scratch paper, and any additional
allowable reference sheets that are given to
the students to use during testing are
considered secure material. Examiners are to
collect them and count them to verify that all
have been collected from the students before
the students leave the testing room. This is to
prevent students from writing secure
information on the paper and carrying it out
with them. These papers must be collected
and destroyed by the Principal/Building Level
Coordinator at the end of each testing session.
Make-up Sessions
• The Principal/Building Level Coordinator is
responsible for scheduling the time and place
for the make-up sessions and for assigning an
Examiner to administer the make-us tests.
• All test security and test administration
procedures must be followed before, during,
and after each testing session.
• Follow all scripted directions during the makeup sessions.
• Give the students all scheduled breaks and
monitor during breaks.
Preparing Students for Testing
• Ensure all students are familiar with both the
general types of questions on the test and the
procedures to follow when recording the
answers.
• Remind students the day before the test to
bring appropriate activities to work on when
they complete their tests.
– Have quiet activities prepared for those students who
forget to bring their own. Activities must be unrelated to
that day’s testing.
Before Test Administration
• Obtain the testing materials from the Principal/Building
Level Coordinator.
– secure test booklets and Answer Documents that must be
checked out and recorded on the Security Checklist
– a “DO NOT DISTURB” sign for the door

Before Test Administration
• Ensure you have a copy of the Test Examiner’s
Manual , as well as manipulatives and reference
sheets, as needed for each student.
• Check each test booklet (Grade 3) or Answer
Document (Grades 4-11) to verify that it has a
pre-coded student label. If there is no pre-coded
label and the Bio-Grid is blank, contact the
Principal/Building Level Coordinator.
• Leave the student name and teacher name lines
blank on the front cover of the test booklets.

Preparing Manipulatives
• Mathematics punch-out rulers for grades 3, 4, 5, 6, 8,
10, and 11 are provided with the testing materials
and should be prepared prior to the day the
mathematics test is administered.
• Students may also use personal rulers or calculators,
as long as these tools do not display formulas, hints,
or information that could be used to answer test
questions.
• Mathematics Formula Sheets (grades 9 –11), Periodic
Tables (grades 5-11) and Science Reference Sheets
(grades 7-11) are separate from the test booklets and
must be distributed with the math and science tests,
as applicable.
Preparing the Classroom for Testing
• Clear desks and work areas of all booklets,
papers, and other materials.
• Ensure that NO information pertinent to the
test being administered is displayed in the
classroom during test administration.
Preparing Calculators
• Any kind of graphing calculator may be used,
except one with a typewriter-style (QWERTY)
keypad or one that includes a computer algebra
system (CAS).
• All types of memory—including standard
memory, ROM, and flash ROM—must be cleared
to factory defaults both before and after testing.
• Any programs or applications must be removed
prior to and after testing.

On the test booklet cover for grades 3 through 11:
• during the first test session, ensure each student
prints his/her name on the front cover of the test
booklet.

• When Session 2 begins, make sure the item
number in the test booklet matches the item
number on the answer document.
Script for Administration
• Examiners must follow the scripts verbatim
for each content test.
• Reading/Language Arts tests may not be read
aloud or signed to any student. Only the
directions may be read aloud or signed.

– Make sure you are reading the script that matches

Mathematics
Day 1: Session 1, Mathematics
Distribute the test booklets, scratch paper, graph paper, a set of mathematics punch-out tools
(manipulatives), and calculators to each student. (Calculators are not required and are not necessary
for taking the test.) Make sure that every student gets his/her own test booklet.
Students may use scratch paper on the Mathematics test.
Check to make sure that your name is on the pre-printed label on the cover of the test
booklet.
SAY
If the information on the label is incorrect, please raise your hand.
On the front cover of the test booklet, write your name on the student name line. On
the Teacher Name Line, write my name above the M.
Demonstrate on the board how the students are to fill out the Teacher Name Line. Check to make
sure all students are completing the lines correctly.
The Mathematics test has two sessions.
SAY
Mark your answers in the test booklet by filling in the circle beside each answer you
choose. You may use the scratch paper to work the questions.
If you make a mistake when you fill in the circle, make sure you erase it completely.

Reminder: The Reading and Language Arts test may not be read aloud or signed to any student.
Only the directions may be read aloud or signed.
Reading and Language Arts
Day 2: Session 1, Reading and Language Arts
Distribute test booklets and answer sheets to the students. Make sure each student gets his/her
own test booklet and answer sheet. Students may use scratch paper on the Reading and Language
Arts test.
Check to make sure your name is on the pre-printed label on the cover of the answer
sheet. Look at the boxes labeled Security Barcode in the upper right corner.
(Pause.)
SAY
Now, find the Security Inventory Barcode on the front of your test booklet and make
sure these numbers match and are filled in correctly. If you need help, please raise your
hand.
Look at the Teacher Name line on the front cover of the test booklet. On this line, print
my name above RLA.
Demonstrate on the board how students are to fill in the Teacher Name line. Check to make sure
all students fill in this line correctly.
During Testing
• Set aside the test booklets and answer documents
for students who are absent.
– Each day, shortly after the test administration begins,
the Principal/Building Level Coordinator will collect
• test booklets (ALL grades)
• answer documents (Grades 4-11)
• Read the student directions verbatim.
– Directions to be read aloud are printed in boxes and
preceded by the word SAY.
– Explanations providing greater detail are printed in
italics.
During Testing
• Give students adequate time to complete the
content area test.
– If a student requiring additional time stops working
for more than 10 minutes, it is recommended that the
testing session for that student be terminated.
• Each content area test must be completed on the
day it begins.
• Circulate around the room to monitor student
progress on the test and to make sure students
are doing their own work.
During Testing
• Record any test administration problems on the
WESTEST 2 Testing Irregularity Sheet (see
Appendix J).
– All irregularities that could lead to invalidation of the
test results must be reported to the Principal/Building
Level Coordinator as soon as possible on the same day
they occur.
• Damaged test booklets (All Grades) and answer
documents (Grades 4-11) need to be reported to
the Principal/Building Level Coordinator
immediately.
During Testing
• Do not destroy or dispose of damaged test
booklets or answer documents.
• Grade 3 - Make sure students write in their
test booklets only in approved areas. Students
should not write close to the answer bubbles
or near the timing marks on the edge of the
page. Students should fill in each circle
completely for their answer choices.
• Students who complete a test early should
close their test booklet and answer document.
– Do not re-open until instructed to do so.
During Testing
• Make sure test booklets are closed and test
materials are secure during breaks.
• At the end of each day of testing
– check in all test booklets and answer documents to
the Principal/Building Level Coordinator.
– ensure the number of test booklets and answer
documents matches the number assigned to you on
the Security Checklist.
– return all scratch paper, graph paper, formula sheets,
etc., to the principal or building coordinator
After Testing
• Sort the test booklets in numerical order by Security
Barcode, with the spines facing in the same direction.
• Sort the answer documents in order matching the
numerical order of the test booklets.
• Return the test booklets and answer documents to
the Principal/Building Level Coordinator at the
locked and secure central location immediately
after the testing session has ended.
• Wait for the materials to be counted and signed for
by the Principal/Building Level Coordinator.
After Testing
• Return the completed WESTEST 2 Testing
Irregularity Sheet to the Principal/Building Level
Coordinator if an irregularity has occurred.
After Testing
• Return the secure scratch paper and graph paper to the
Principal/Building Level Coordinator for shredding.
• Verify the Braille bubble has not been filled in
unnecessarily.
• Verify that the box labeled “Admin Use Only–Invalid” on
the inside-front cover of the test booklet (Grade 3) and
front lower right corner of the bio-grid page in the
answer document (Grades 4-11) has not been filled in
unnecessarily. Only the Principal/Building Level
Coordinator may invalidate a content area test after
obtaining approval from the county test coordinator.

Scenario
The Grade 5 Examiner followed the script to
begin the RLA portion of WESTEST 2. After
she was confident her students understood
the directions, told them to begin. While
walking around the room, the teacher noticed
one student bubbling answers in the Social
Studies section of the answer document. The
student was directed to re-mark the answers
in the RLA section of the answer document.
3.48. Testing Code of Ethics. The Testing Code of Ethics, as set
forth in this policy, defines the moral and principled practice
of handling, training, and administering secure assessment
materials.

5. Examiners shall limit assistance to students to only those
issues concerning the mechanical aspects of marking
answers, clarifying directions, and finding the right place on
answer sheets. Examiners shall not indicate answers, point
out the rationale of an item, or prompt students in any
manner.
Scenario
A teacher locked her room and took the
students to the water fountain for the break
between Math session 1 and 2. While at the
water fountain, the teacher started yelling at
the students that they could do better than
they were doing on the assessment.
3.43 Security Breach. A security breach is the failure to
observe/follow the documented procedures established to
protect, maintain and implement the testing process, such as
the Testing Code of Ethics, test procedures agreements,
and/or procedures prescribed in the testing manuals by any
person administering or assisting with the administration of
an assessment included in the WV-MAP.
Appendix A--Testing Code of Ethics
1. County school personnel shall establish and implement
procedures to ensure maximum test security and limit access
of secure materials to applicable county/school personnel.
Scenario
The Principal/BLC prepared student rosters so
that the special needs students would be tested
by the teachers who normally provided their
accommodations. The group’s IEP listed oral
testing and fewer answer choices as an
accommodation, so the teacher/Examiner
provided those accommodations on WESTEST 2.
3.47. Testing Administration Breach. A testing administration breach is any failure
to follow the documented test administration procedures that could alter the
results or testing environment of the assessment for one or more students,
e.g., timing an untimed test, not implementing required accommodations, etc.
See §126-14-7.7 for the protocol to use in reporting an alleged breach.
4.3. All public school students with disabilities who are eligible for services as
defined in Policy 2419 shall participate in the WESTEST 2 or APTA in the grade
level at which they are enrolled with appropriate accommodations, if any, as
determined by their IEP Team.
5.6. All allowable testing accommodations identified on the student’s IEP, Section
504 Plan, or LEP Assessment Participation Form must be provided during the
test administration, as established in his/her plan. Students needing
accommodations will be identified by the Principal or Building Level
Coordinator and the designated accommodations shall be implemented by the
examiner.
Scenario
Students are being tested in the library. The
librarian had decorated the lab’s bulletin
boards prior to the test with posters that
contain punctuation rules. The posters are not
taken down or covered up prior to testing. On
the day of the test, Mr. Gibson, the Examiner,
notices the posters but decides not to say
anything.
3.10. Cheating. Cheating is a willful or intentional act of
dishonesty by an administrator, teacher, examiner, or student
involving preparing for tests, test taking, altering records or
performance that impacts and/or jeopardizes the integrity of
the test or the accuracy of the test results. Cheating will result
in the invalidation of test scores and tests as per the
recommendation of the Investigative Team and the
determination of WVDE. Cheating may result in a loss of
licensure/certification for employees as per §126-14-8.7.1

Any written or oral report alleging a violation of
test security shall be investigated. If the
investigating team determines that violations
have occurred, appropriate action shall be taken
which may include:

Accommodations for Students with
an IEP or Section 504 Plan
• All students with an IEP or a Section 504 Plan
are to participate in all components of the
WESTEST 2 in the grade level at which they
are enrolled, except for those students who
meet the criteria for participation in the West
Virginia Alternate Performance Task
Assessment (APTA).
• It is recommended that students with an IEP
or a Section 504 Plan be assessed following
the same testing schedule as their regular
education peers in their enrolled grade.
• Students with an IEP or Section 504 Plan
should be tested in the same content area on
the same day as their peers unless there are
extenuating circumstances and prior
approval has been obtained from the County
Test Coordinator.
• The student is to receive all assessment
accommodations as outlined in the IEP or
Section 504 Plan. For additional information,

• If a school does not provide the
accommodations listed on the IEP, the results
may be invalidated and the student will be

accommodations, he/she is in violation of the
IEP which is a legally binding document.
Accommodation
An accommodation is a change in the
administration of an assessment, such as
setting, scheduling, timing, presentation
format, response mode or others, including any
combination of these, that does not change
what is intended to be measured by the
assessment or the meaning of the resulting
scores; an accommodation does not change
the construct of the test.
Presentation
• Use sign language to present directions,
stimulus material, questions and/or answer
choices for the Mathematics, Science and
Social Studies tests.
• Sign ONLY the directions on the Reading/
Language Arts test.
• Read aloud verbatim the directions, stimulus
material, questions and/or answer choices for the
Mathematics, Science and Social Studies tests as per
the IEP or Section 504 Plan.
• On the Reading/Language Arts test, only the directions
may be read aloud.
• For a blind/partially-sighted student who does not read
Contracted Braille or Nemeth Code, the directions,
stimulus material, questions and/or answer choices may
be read aloud verbatim to the student for the
Mathematics, Science and Social Studies tests.
• Use a text-talk converter to present directions,
stimulus material, questions and/or answer
choices verbatim for a blind/partially-sighted
student, when that is the student’s typical
mode of accessing written material.
• Rephrase directions to state the same
message in a new or different way and not
breach security of the test items or give away
an answer. (ONLY by a trained Examiner.)
• Use Braille or other tactile form of print
when that is the student’s typical mode of
accessing written material. Student
responses must be transcribed to a standard
test booklet (Grade 3) or onto an answer
document (Grades 4-11). Refer to Directions
for Transcribing in the Examiner’s Manual.
• Use an electronic translator or sign-dictionary
to present directions, stimulus material,
questions, and answer choices for the
Mathematics, Social Studies, and Science
tests.
• On the Reading/Language Arts test, only the

translator or sign-dictionary.

manager.

Any necessary changes to the WVS.326
should be submitted to the county prior to
testing. A new WVS.326 must be printed if
changes are made.

The WVS.326 document used to verify that
accommodations were provided must be the
most current, up-to-date version available.

3.25. Locked and Secured Central Location/s. A locked and secured
central location/s for storage of secure materials is any location that
can be locked and secured with access limited to only the Principal
and/or Building Level Coordinator.
5.3.2. Any West Virginia public/nonpublic school employee involved
in any phase of the testing program shall be trained in secure
materials and/or assessment administration procedures as defined
in policy.
7.3.1. Apart from the scheduled test administration, the County Test
Coordinator shall ensure that secure test materials shall be stored in
a locked and secured, central location by the County Test
Coordinator consistent with test security procedures outlined in the
Testing Code of Ethics (Appendix A).
Scenario
Johnny moved to WV and enrolled in school
on the last day of WESTEST 2 Online Writing
window. His principal is now telling his parents
he cannot take WESTEST 2.
He can take the remainder of WESTEST 2 if he
has not taken the online writing. His R/LA
score will be lower because he did not have
the opportunity to earn any points for that
part of the test.
