Brief description of incident
At about 14:00 on March 6, 2015, one mechanical crew’s pickup (T37) from crew 9811
went back to camp after completing a field repairing work. Before the pickup passing a Tjunction, one local truck coming from opposite direction turned left suddenly to the soil road
of the T-junction without applying indicators and blocked the way of T37 fully. The driver of
T37 used brake immediately, but the short distance (only 15m) was not enough to stop the
pickup completely. Finally, the pickup collided on the rear right side of the local truck. After
investigation, the traffic policemen judged that the local truck was full responsibility.
No injuries happened in both vehicles. The front part of T37 was distorted. The rear right
side of local truck was cracked and scratched slightly. The total estimated loss of T37 was
about 2000USD.

Causes:
 The local truck turn left suddenly and occupied the driving lane of T37.
 The indicator of the local truck was not work.
The light rain cause the road was slippery.
 T37 driver didn’t reduce speed before passing the T-junction.
 Both of the two drivers are lack of defensive driving awareness.
Lessons Learned :
 Be alert with the local drivers’ unsafe behaviors.
 Reduce the speed before T-junction.
 Observe the MOPO procedure while driving in special weather.
 Improve the defensive driving awareness of the crew drivers.

Brief description of incident
At 11:53 on March 19, one subcontractor’s Toyota pickup working for BGP GIS project
drove 62km/h on the blacktop road with two surveyors. One local blue water tanker from
the opposite direction turned to left lane suddenly and rushed toward the Toyota pickup. In
order to avoid collision, the pickup driver turned right emergently, which cause the pickup to
roll over out of the road. After the accident, the local tanker was escaped.
No injuries happened for the driver and passengers. The windscreen was broken, one tyre
was flat and the rear-view mirrors were damaged. The total estimated loss was about
2400USD.

Causes:
 The local tanker turned to left suddenly without any indicators.
 The GIS pickup driver didn’t respond properly.
 The GIS pickup driver was lack of the defensive driving awareness to the
local truck.
 The crew didn’t test or assess the driving skill of the subcontractor drivers.
Lessons Learned :
 Be alert with the local drivers’ unsafe behaviors on the main road.
 Improve the drivers to emergency response capability depending on the
potential cases.
 Test or assess the competence of the drivers from the subcontractors.
 The subcontractor’s drivers should receive DDC training as well.
