Overview
• To provide an overview of Personalisation
within the national context of TLAP and
MIR in 2012.
• Highlighting how My Choice Marketplace
responds to the key strategic priorities
detailed in TLAP and MIR.

About the TLAP Partnership
• The sector-wide partnership agreement was launched in November
2010. States the commitment to moving forward with personalisation
and community-based support.
• More than 30 national and umbrella organisations representing the
broad interest in transforming adult social care across England have
committed to working in partnership.
• The organisations who joined the partnership will work collectively to
develop and share innovative practice so all citizens have more
choice and control over the support they experience.
• Picks up where Putting People First left off but importantly
represents a key shift from government-led to sector-led support.

• Principles of Personalisation, Community Based Care
and Support at centre
• Focus on effective prevention, shifting resources from
crisis and acute interventions
• Overall aim to secure a shift where as many people as
possible are enabled to stay healthy and actively
involved in their communities for longer and delaying or
avoiding need for targeted services
TLAP Priority Work Streams
• Six priority areas for action
– highlighting the importance of co production.
– personalisation and personal budgets
– developing cost effective and efficient
solutions
– developing the provider market and workforce
– building community capacity
– improving information to the public
Making It Real
• Markers used to support
all those working towards
personalisation
• Supporting organisations
to check their progress
and decide what they
need to do to keep
moving forward to deliver
real change and positive
outcomes with people
What is Making it Real?
• A set of “I” statements from people who use services ,
carer and citizens saying what they would expect to see
and experience if personalisation is real and working well
in a locality or organisation.
• These are then matched against key elements that
would need to be in place to make these experiences
possible.
• These “markers” will help show how well a locality or
organisation is doing in achieving personalisation and
community-based support.

• 1) Information and Advice: having the information I need, when I
need it
• “I have the information and support I need in order to remain as
independent as possible.”
• “I have access to easy-to-understand information about care and
support which is consistent, accurate, accessible and up to date.”
• “I know where to get information about what is going on in my
community.”
• IN PRACTICE... Trusted information sources, are established and
maintained that are accurate, free at the point of delivery, and linked
to local and community information sources.

Personal budgets and self-funding: my money
“I can decide the kind of support I need and when, where and how
to receive it”.
“I know the amount of money available to me for care and support
needs, and I can determine how this is used (whether its my own
money, direct payment, or a council managed personal budget).”
IN PRACTICE... There is a market of diverse and culturally
appropriate support and services that people who use services
and carers can access. People have maximum choice and control
over a range of good value, safe and high quality supports.
Self-funders receive the information and advice that they need
and are supported to have maximum choice and control.
Trafford and Making it Real
• Trafford signed up with TLAP to be a test
site prior to National Launch in May 2012
• Co-produced self assessment and
developing action plan with Citizen
Reference Board.
• Involved in Regional Launch on 14th May
• Working with providers across all of the
provider markets in Trafford, encouraging
commitment to Making it Real
My Choice Marketplace
• Trafford Council is committed to progressing
Personalisation and delivering the strategic priorities
defined in Think Local, Act Personal and Making it Real.
• Commissioned the development of My Choice
Marketplace to respond to the need for a range of
innovative, flexible services that are available to both
personal budget users and self funders.
• Encourage citizens of Trafford, professionals and
providers to use My Choice Marketplace, to access a
range of good quality, diverse, responsive services.
