• Helps in writing evals
Blocks 1 & 2
• Block 1: Member’s Name
– ALL CAPS
– LAST NAME, FIRST NAME, MI
• If no middle name put NMN
– Example: NEMECEK, WILLIAM J
• Block 2: Rank
– Drop down menu
– Select appropriate rank
Blocks 3 & 4
• Block 3: Company
– Drop down menu
• Select appropriate company
• Select platoon/squad
• Block 4: Occasion for Report
– Midterm
• Check midterm box only
– Final
• Check Fall
Blocks 5 & 6
• Block 5: Date of report
– Format: YRMONDAY
• Midterm: 08OCT24
• Final: 08NOV26
• Block 6: Type of Report
– Check the “Regular” box
• “Special” only used for MOM, ADOM, performance
counseling
