UU CSW HPPAE Program:
University-Community Partnership
 Our HPPAE Program started in 2005.
 The University of Utah College of Social Work houses a
service arm, the Neighbors Helping Neighbors Program
(NHN), which is the Host Site for HPPAE students.
 In addition to fulfilling the basic requirements of a 2nd year
MSW practicum, HPPAE students complete
 Specialized mini-rotations
 Masters Program Vital Aging Groups in Senior Centers
 Caring Connections Grief Groups in the College of Nursing
 Bereavement Study in the College of Nursing
 A rotation within a community agency






Adult Day Services
Nursing home
Local and State government
VA Medical Center
UU CSW HPPAE Program:
Communication with Partners
 LCSWs at NHN are the primary field instructors and
they coordinate learning experience with community
agency instructors and task supervisors.
 Students participate in weekly supervision to discuss
rotations and receive feedback and support from NHN
supervisors.
 NHN LCSWs meet quarterly with rotation sites to
evaluate student performance, discuss problems and
solutions, discuss successes.
 Field instructors from community agencies are invited to
present in the Interdisciplinary Seminar Series Spring
semester.
UU CSW HPPAE Program in 2012
 NHN provides stipends and tuition waivers.
 Funding for stipends from local private foundations helps
us be competitive.
 We focus on advanced practice gero social work
competencies and use the Geriatric Social Work Scale II
for pre-post evaluation.
 We promote NHN practicum and SW in Aging track
on the CSW website, in application materials, at track
selection workshops and practicum fair, and through
promotional activities with food.
 We work with faculty to incorporate aging content into
foundation courses.
 Dean Jannah Mather is extremely supportive.

New Opportunities:
VHA/GRECC and HPPAE
 The John A. Hartford Foundation, the
VHA and SWLI have partnered to create
VHA/GRECC HPPAE field placements
for MSW students.
 The model is being piloted in Salt Lake City,
Birmingham and Madison VAs. Dr. Fran
Wilby and I are serving as mentors for the
local SLC VA/GRECC site.
