What is The Expert Patients Programme?

    A new approach to managing long-term health conditions A free 6 week self- management course of two and a half hours per week Courses are delivered by two lay voluntary tutors Tutors will a have experience of a long term health condition

What is The Expert Patients Programme?

   EPP does not provide clinical information or treatment, nor does it look at clinical needs.

The aim is to give participants the confidence to take responsibility for their own care, while also encouraging them to work in partnership with health and social care professionals.

It is based on research evidence from the US & UK.

Who can attend a course?

 Anyone living with a long term health condition  People decide for themselves if they want to attend a course -----Self referral  All interested people are added to a Database  All potential participants are informed of coming courses

Who developed EEP?

 Stanford University - Professor Kate Lorig & colleagues in 1970’s  Chronic Disease Self-Management Programme (CDSMP) developed mid 1990’s  Copyright belongs to Stanford University Welsh Assembly Government have a licence  Course delivered to a script

  "I thought it was a friendly, happy and relaxing course. I had spent a month in Whitchurch hospital with clinical depression and although i was much better, the course gave me that extra confidence “Admittedly on the first morning I almost decided to cancel, as it felt like too much effort, but I forced myself to attend and was glad afterwards that I did. I made new friends and found a group of people with varying illnesses/afflictions, enduring the same negative emotions and symptoms as myself, including the tutors! The latter fact shocked me into paying attention as I felt that our trainers properly understood what we were going through.

Looking After Me (LAM) A course for Carers

Looking after me (LAM) is a course for adults who care for someone living with a long term health condition or disability The LAM course is provided free to any carer in Wales who wishes to attend

Looking After Me

The LAM course aims to:  encourage the carer to take time for themselves and to look after their own health needs.  help them as a carer to take more control over their situation and make a difference to the quality of their life.

What does it cover?

       Relaxation Techniques Dealing with tiredness Exercise Healthy eating Coping with depression Communicating with family, friends and professionals Planning for the future
