TDaP Requirement

Who? All incoming 7th graders must have TDaP
immunization prior to enrollment.

Why? This immunization is required by the Ohio
Department of Health.

What? It protects kids from Tetanus, Diphtheria, and
Pertussis (Whooping Cough.)

When? At age 11 and 12 this shot will show up on
your physician’s immunization list. Please send a
copy of this immunization record to KMS.
