•59 Patients reviewed
•9 Prescription changes made and 4 referrals to the Community
Dietitians
CUMBERLAND CARE HOME:
•49 Patients reviewed
•13 Prescription changes made and 3 referrals to the Community
Dietitians

•108 Patients reviewed
•22 Prescription changes
•7 Referrals to Community Dietitians

Patient X:
78 yr old female
Wt=53kg Ht = 1.57m BMI=21.5kg/m (ideal weight)
Lost 4kg in past 4 months = 7% weight loss
Rx: Calogen 30ml tds since hospital visit 6 months ago
At Risk, needs GP assessment and dietetic review as losing
wt and Calogen not adequate
