NDP, provides services including counseling, records in

find out who else needs services and should be
followed-up by the CHT Partner

master listing, giving
of messages and
accomplishing of
Family Health Record
Form
NDP checks the entries in the
LOGBOOK and helps the CHT
Partner in:
• Correctly filling-out the
logbook, Family Health
Record Form and Masterlist
• Polishing the messages
Tool of the CHT
• CHT Partner: Columnar Notebook or logbook
including Family Health Record Form
