Date of program enrollment in MM/DD/YYYY format
Site Code
‘1’ for single site providers. Approved site codes can be downloaded
from File Download Page.
Delivery Modality
Face-to-Face (1), Online (2), Hybrid (3)
Degree Code

Field Code and/or
Endorsement Codes
Field Code A is required for all initial certification programs.
