If you are doing green days, you may turn these into the coach. If not, turn into Activities Office.

The student signs in two places, parent signs in three. Make sure Dr. has signed and DATED page 3. If you had your physical done at school, we will have that on record.

Bring a copy to Activities Office (even if you’ve already turned it into a coach).

School Accident Insurance may be purchased prior to the start of the season. As soon as we know the name and contact information of this coming year’s vendor, we will post it.

# 4: CONCUSSION EDUCATION PROGRAM

• • • Student and one parent must complete this online seminar. You do not need to print anything when you are done as your information automatically goes into our system. DO NOT CONFUSE THIS WITH THE IMPACT PROGRAM used by our Certified Athletic Trainers to measure baselines on each athlete.
