• Documentation of Allergies and Reactions
• Completion of Medication Reconciliation, if applicable
• Physician order to accompany all administered meds
• Use of unapproved abbreviations

Early Findings
• Unapproved abbreviations most commonly used:
• “cc” is being used instead of “ml”
• “qd” being used
• Continued use of AM/PM rather than 2400
• Documentation of allergies, but no documentation
of sensitivities
