ADE Documentation Requirements for PHLOTEs
Language Acquisition Department (LAD) 2013-2014
Objectives
• The participants will be able to maintain PHLOTE student documentation for ADE compliance by : – Identifying PHLOTE students – Organizing forms needed for documentation based on student ’ s status – Maintaining documentation records as an on going process
Place the student in the M ainstream program or, if requested, in Dual Language (DL).
PHLOTE PLACEMENT FLOW CHART Check the responses to the three language questions on the Registration Form and Home Language Survey I f all three responses are “English”on both forms … Administer AZELLA I f the response to one or more questions on either form is other than “English”... I f student scores becomes
