• Salvage cryotherapie na radiotherapie
• Ziekte controle na recidief

1.Extracellulair ijs-cristal-formatie (<-7°C)
→terugtrekken van water hyperosmotisch extracellulair gebied
→ water is onttrokken aan cellen
→ cell krimpen, membramen en cellen worden beschadigd
door dehydratie

2. Intracellulair ijs-cristal-formatie
(<-15°C en snel (!) bevriezing ~25°C/ min)
→ laat langzaam osmotisch proces niet toe
→ ijs cristallen reageren
→ verstoring cel functie en cel membraam

3. Vasculair effects
•Tgv vasoconstrictie→ circulatie vermindering
→ formatie micortrombus→ cellulaire ischemie en necrose
•Vasculaire schade

Bevriezing: zo snel als mogelijk
Temperatuur
minimaal -40°C, omdat extra- en intracellulaire
ijsformatie niet wordt bereikt bij hogere temperaturen
Cryo Therapy
Tot 25 naalden afhankelijk van de volume prostaat
Naalden worden geplaatst onder transrectale echo
Twee bevriezings cycli snel naar -30°C, en temperatuur
10 minuten vasthouden.
Temperatuurmeting 2-5 onafhankelijke sensors

Percutane plaatsing naalden onder laproscopie en
echo maar ook onder CT
Twee cicly snel bevriezing tot -30°C, ongeveer 10
min, onderbroken door ondooien
