Requisition Form
•The equivalent of a really good grocery
list.
•Proof of what you are planning to
purchase.
•Can be referenced again in the future.
This is an example of how to fill out the requisition form.
In order to determine the commodity code for your items, follow the link
on the next slide.
If you will be utilizing more than one store, fill out a requisition for each
store.
(You are not personally buying anything, I just want you to go
shopping...at the store or online, up to you)
Remember, you will need to order these items:
20 #8 zinc coated 1 ½” wood screws
1x8 yellow pine (you determine length)
1” round poplar dowel (you determine length)
varnish or stain (you determine amount and type)
