What is an FQHC ?
• Grantee under Section 330 of the Public Health
Service Act, mostly Section 501(c)(3) tax exempt
non-profit entities, but can be a public grantee
(county health departments, etc.)
• Covered under FTCA and qualify as a covered
entity under the Section 340B drug discount
program
• Defined as FQHCs in the Medicaid and Medicare
statute – also includes “look-a-likes”

• Founded by a nurse practitioner in 1997
• Managed by Nurse practitioners operating
under an advanced practice nursing model

Mission
To operate and maintain nonprofit clinics and
other related activities encompassing the whole
person –physical mental and spiritual with a
focus on health education that includes illness
prevention, health maintenance and wellness.
Vision
For all residents of East Feliciana Parish and
surrounding parishes to have access to
affordable quality health and wellness services,
enabling them to achieve quality of life for
themselves and their families.
A Meager Beginning: A Fabulous
15 Years of Growth
• March 1999 opened RKM as a licensed Rural Health Clinic
(RHC) in 800 Sq ft 50 yr old Jim Walter Home
• August 1999 Clinton Middle Health Center
(only licensed RHC School Based Health
Center in the state) Goal - sustainability
• PCPFHF board developed a ten year Strategic Plan; Goal: to
secure land/ permanent housing by 2001
• 2002 Received USDA government secured loan to construct
new 3600 sq ft state of art center on 5.7 acres. Opened April
2003
• 2005 Look Alike status and FQHC New Access Funding ;
The Storms – blew in foundation grants -$250,000 opened
2nd School Based Clinic
• 2007 NAP West Baton Rouge opened 1/5/2008
• 2008 NAP Clinton Lower Health Center and Slaughter
Health Center opened 2009
• 2009 Received Capital outlay money from State l and
leveraged state and Federal New Market Tax Credit
loan
Constructed 19000 sq ft state of art medical complex to
house RKM Primary Care; Renovation of old building to
house Behavioral Health; Construction of 6000 sq ft
Administration /Education Building
• Completion of the second 10 year Strategic Plan
Special Populations - Services
• Primary and Preventative care for Children Adults
throughout the lifecycle.;
• Prenatal and Postpartum services, with OB By
referral.
• Preventative / basic restorative dental services with
limited specialty services
• Podiatry
• Integrated behavioral health with bereavement
substance abuse, psychiatric and psychological
services,
• Tele radiology, and tele health capabilities for
psychiatry, dermatology ophthalmology, cardiology,
and primary care

• School based health – full service clinic
located on every school campus in East
Feliciana Parish (open to community)
• Bereavement counselors - responsive to crisis
related needs of schools in East Feliciana, East
Baton Rouge, Ascension, Tangipahoa and
Livingston Parish
• HIV and Hepatitis C early screening prevention
and treatment program

• RKM - main campus - is open from 7AM to 8 PM Monday
thru Thursday; 8 AM to 5PM Friday and 9AM to 1PM on
Saturday.
• WBR Primary Care -8AM to 5PM Tuesday, Thursday Friday
8AM to 8PM Monday and Wednesday
• RKM Primary Care Livingston -8AM to 5PM Monday thru
Friday
• All other sites are open a minimum of one hour before
hours or on hour after hours.
• EHRs allow pts to access care at any site
• 24/7 provider manned on call services.
• Open access network - we perform today’s work today.
Same day appointments are available - exception

• Smoking cessation – smoke free campus
• East Feliciana and WBR parish schools are
certified as asthma friendly environments.
• Be Fit and Health Wise – six week program held
annually in the summer for ages 6-18
• Winning Weigh -6 week program for adults
inclusive of nutritional education, physical fitness
• Free HIV and Hepatitis C screening for every
patient unless they opt out.

Standards
FQHCS are:
• Held to the highest administrative, clinical, and
financial performance and accountability
standards
• Complete a budget renewal (non compete)
application every year for two years.
• Compete for continued funding through Service
Area Competition (SAC) every 3rd year
• HRSA Site Visit is incurred during the year of the
second budget renewal.

benefits for low-income communities in 2009.
• Produced nearly 190,000 jobs that same year.
• Will create 284,000 new jobs and generate
$54 billion in overall economic benefits by
