Incident details
On the morning of the 14 th October 2010, a Northbound “Western Star” Prime Mover (Registration 216 LYQ) and “B” double trailer was reported as having rolled into the table drain within a RoadTek roadwork project.
Marks left in the recent works indicate the heavy vehicle had run over the edge of the left shoulder some distance prior and in the process of “righting” the path, the truck has rolled onto its left side in the left table drain.
A following heavy vehicle took evasive action to avoid a collision and had become “bogged” in the RHS drain. Once assistance was given to this heavy vehicle, it continued on its way. No injuries have been reported at this time.
The northbound truck would have driven past the following roadwork signs At 3.2km Roadwork Ahead Signs on Left and Right Hand sides mounted on posts At 1.2km Traffic Hazard Ahead Sign on Left Hand side mounted on Star pickets with reflector and flashing amber light At 280m, a multi message sign on Left and Right hand sides with the combination of Road Work Ahead / 80K / Reduce Speed (with the 80K panel is closest to the road) On arrival at site, a video run of the project was completed, images taken of the signs in place, and traffic control assistance given to Police.
Some of the heavy vehicles load (approximately 1000L of engine coolant) had escaped from the truck and the drain was blocked to prevent contamination of the local waterways.
Once the vehicle was removed, the site was cleaned, and made safe.

Discussion
Approach to the accident site from the south is a straight o EFFECT – Good straight road with excellent visibility New road works o Good first coat prime/seal on Stabilised Pavement, with Temporary Raised Pavement Markers every 60m o Edge delineation provided with Star Pickets and reflective caps Nil reports of other drivers being involved prior to accident from Police at site
Causal Factors :-
The lack of skid marks prior to the heavy vehicle running over the shoulder could indicate there was no evasive action being taken. ( Suspect driver fatigue, inattention or distraction.
) Root Cause :-
Unknown – Police Matter
Summary
The approach to the accident site is clear, straight, sealed and level. Advance warning signs were erected with temporary raised pavement markers in place.
Recommendations
1.
Continue to assist Police in accident scene traffic management 2.
Informing of internal accident investigator as early as possible of traffic accidents at road work sites

What We Need You Thinking About • Safety First - Your Own / Others / 000 / Report up.
• Secure the site / When Safe - Organise - Delegate if in Charge / Protect site from further accidents.
• Start Looking and Think – Photos / Video / not just of the big things. Skid Marks / Indentations / Delineation / Surface condition.
• Look for things that may have been a Cause / Factors / Write notes – Time / Conditions / Angles – shoot from all Different Angles but know which way is which / Write notes against each photo if possible / drop offs.
• Try to get information off Police – Report Number / Officers Name and Rank / Where is he based / Who is Lead Investigator.
Main Message • Above all don’t judge – Not our business to make accusations / judgement on who is right or wrong.
Main Message • Our duty is to collect evidence that will protect not only ourselves but RoadTek and TMR.

• Horrifically, the twins were confronted with the devastating news their brother Bobby, 17, had been killed in a triple-fatal accident.
• He and two friends died when the car they were in crashed in heavy rain in Sydney's west early yesterday.
• "I didn't get it. All these people were writing, 'RIP Chris Naylor' and 'RIP Bobby', and I thought: 'What's going on?'," Angela said.
• Desperate for reassurance there was a mistake, she tried over and over to ring her brother.
• "I kept ringing and messaging but couldn't get on to him. So I rang Mum and said: 'Chris Naylor must have died - I just read it on Facebook. But where's Bobby? People are writing 'RIP Bobby' too.' • "Mum said 'Bobby was with Chris Naylor last night'."

• Is Faster then a PO can get to the home.
So What Can We Do and how • Protect The Site from visual Gawkers.
• Block the view.
• Put up Anti Gawk Screens.
• Prevent our own staff from posting.
• Talk to crews prior.
• Park Machines in a half moon configuration.
• Make anti Gawking Screens out of Geo fabric / Temporary fencing.
• Supply Tarps.
Project Sites • Think about your surroundings.
• Protect the injured – they are family.
• Put in Anti Gawking protection – remember serious accidents take hours sometimes to clear or resolve. • Control Media Intrusion.
• Support the Police.
• Remember once safe – take control of the site make things happen / record events.
Session 4 – Fatigue Management • Multiple Times throughout the year we get caught in circumstances outside our control.
• But we need to address the event and protect our staff from Injury.
