Companion to the Epi Info™ 7 suite of tools

Allows the survey designer to collect information from
participants over the internet.

Data collection only – not a data management tool

Survey forms can be published to any properly configured
web server hosted by your institution or an outside party

Specific Universal Resource Locator (URL) or website address
is created when published.

Distribute the URL over email, by posting it on a web page,
or using other methods.

Participants access the web survey and submit their
responses through a web browser or mobile device.

Responses are downloaded directly into the original Epi
Info™ 7 project for analysis.

Hardware, Software, and Setup is required

Hardware requirements:
• Web server (Windows server 2008 or Windows server 2008 R2)
• Database server

o Internet Information services (IIS) 7.0 / 7.5
o .NET 4.0
o ASP.NET MVC 4
• Database server
o SQL Server 2008
• SMTP Server – application needs to know the name
• Survey Manager Admin Client


Initial Set Up

Requires information that is either provided:
• Web Survey System administrator
• Generated by the Web Survey

Required information:
• Endpoint Address – designates the location or URL of the application
that hosts surveys.
• Organization Key – provides survey designer access to publish the
survey and import the responses.
• Survey Key – identifies the survey that corresponds to the Security
Token for downloading data.
• Security Token – grants access to the survey data and prevents others
with the Organization Key and Survey Key from accessing the data.
Initial Set Up

Settings are configured from the Epi Info 7 Main Menu
• Tools > Options>Web Survey
Designing a Form for Publishing

Attempt to publish a form with non-supported fields…
Publish Form to Web

From the Form Designer menu, select File > Publish Form to
Web.
Publish Form to Web

Publish Form to Web dialog box:
• Introduction Page – provides information to the survey participant
prior to beginning the survey
• Exit Page – provides closing remarks to the survey participant after the
survey is submitted
• Publish Options – specifies technical aspects of the web survey
posting process

Select the survey start and closing dates and times

Click Next to save this information and proceed to the Exit
Page tab
Publish Form to Web

The Exit Page tab displays closing

Select One response only or Many responses allowed
Click Next to proceed to the Publish Options tab.


For first time publishing, Publish Mode = DRAFT
Enter/Paste the Organization Key
Click Publish Form
Publish Form to Web

When publishing completes, dialog shows:

Internet link or URL to send to survey participants
Survey key
Security token

Click the Copy All to Clipboard button.

Paste the information from the Clipboard for later
Test the Draft Survey

Testing the survey is important to confirm:
• field types, questions, and answer choices for analyses
• layout, tab order, skip patterns and other aspects of answer validation

Two ways to test the survey:
1.
Click the ‘Go’ button next to the URL… Launches default browser
2.
Or … Click the ‘Copy’ button,
Launch web browser,
Paste URL into address bar.
Test the Draft Survey




Welcome page is displayed
DRAFT watermark indicates survey mode
Note Pass Code to return to an incomplete response
Click ‘Begin Survey’ to enter a Draft or Test response.
Testing Your Survey

Confirm that you can successfully publish the form
• navigate among the pages
• enter more test data
• reconfirm the validations and Check Code behavior is working
properly.
• check the tab order

To ensure full functionality
• download the test data
• perform a sample analysis.

If an error is observed, make necessary corrections and
repeat these steps until the survey functions as desired.
Import Draft Mode Test Data

From the Epi Info™ main menu:
• Select Enter Data
• Or select Tools > Enter Data

Open the project and form used to publish the web survey.

From the File menu, select Import Data > From Web.

Import responses in Draft Mode,
select one of the “draft mode” import types

Type of Import options:
• Completed records in draft mode Responses include only those
where the ‘Submit Survey’ button
was clicked on last page.
• All records in draft mode Responses include those where ‘Submit Survey’ button was clicked
AND any “Incomplete” responses – such as those where
‘Finish Later’ feature was used.
Import Draft Mode Test Data

Click Import

After the import is complete, click Close.

Responses appear in the survey form and are added to your
Epi Info™ database.

The import process can be done several times as people
begin the survey and responses are created.
