Causes, incidence, and risk

enzyme G-6-PD in red blood cells, causing
destruction of the cells, called hemolysis.

oxidant or stress.

Classification

Alpha (α) thalassemias

connected to the deletion of the 16p

α thalassemias result in excess β chain
production in adults and excess γ chains in

Thalassemia minor (sometimes referred to as
β thalassemia trait) : only one β globin allele
mutation
results mild anemia with microcytosis
Symptoms include weakness and fatigue
most cases β thalassemia minor may be
asymptomatic and may be unaware
Detection usually involves counting the mean
corpuscular volume (size of red blood cells) and
noticing a slightly decreased mean volume than
normal.
Thalassemia intermedia
condition intermediate between the major
and minor forms
often normal life but may need occasional
transfusions e.g. at times of illness or
pregnancy
depends on the severity of their anemia.
Treatment and complications
Thalassemia Major and Intermedia
receive frequent blood transfusions that lead
to iron overload. Iron chelation treatment is
necessary
Untreated thalassemia Major eventually
leads to death usually by heart failure,
therefore birth screening is very important

Acute leukemia

Acute forms of leukemia can occur in
children and young adults
Immediate treatment in acute leukemias
rapid progression and accumulation of the
malignant cells, which then spill over into
the bloodstream and spread to other organs
of the body.
If left untreated, the patient will die within
months or even weeks
Chronic leukemia
excessive buildup of relatively mature, but
still abnormal white blood cells
progress months to years
the cells are produced at a higher rate than
normal cells, resulting in many abnormal
white blood cells in the blood.

Acute lymphocytic leukemia (ALL) :
most common type of leukemia in young
children. This disease also affects adults,
especially those age 65 and older.
Acute myelogenous leukemia (AML) :
occurs more commonly in adults than in
children. This type of leukemia was
previously called acute nonlymphocytic
leukemia.
Chronic lymphocytic leukemia (CLL) :
most often affects adults over the age of 55,
sometimes occurs in younger adults, but it
almost never affects children.
Chronic myelogenous leukemia (CML) :
occurs mainly in adults. A very small
number of children also develop this
disease.
The most common forms in adults are AML
and CLL, whereas in children ALL is more
prevalent
Causes
The exact cause of leukemia is unknown
influenced by both genetic and
environmental factors
from somatic mutations in the DNA which
activate oncogenes or inactivate tumor
suppressor genes, and disrupt the regulation
of cell death, differentiation or division
mutations :
may occur spontaneously or
as a result of exposure to radiation or
carcinogenic substances
exposure to petrochemicals, such as
benzene, and hair dyes to the development
of leukemia
Viruses have also been linked to some forms
of leukemia
ALL are associated with viral infections by
either the human immunodeficiency virus
(HIV, responsible for AIDS)
human T-lymphotropic virus (HTLV-1 and 2, causing adult T-cell leukemia/lymphoma).
Fanconi anemia is also a risk factor for
developing acute myelogenous leukemia

กรณี ถอนฟันหลายซี่ หรื อผ่าตัดเปิ ดเหงือกต้องให้ Factor VIII

- ให้การรักษาทันทีหลังได้รับ factor
- หลีกเลี่ยงการฉีด IM
- LA : หลีกเลี่ยงการฉีด nerve block, ควรใช้
pericemental, intraligamental, intrapulpal,

ให้ FFP ก่อนการรักษา 24-36 ชม. และให้จนได้ระดับ 30 % ของเกณฑ์
ปกติ และให้ทุก 16-24 ชม. ต่อมาถ้าจาเป็ น
Synthetic vasopressin analogue

ทาให้ prothrombin ไม่สามารถเปลี่ยนแปลงเป็ น
thrombin ได้อย่างรวดเร็ ว
ยับยั้งการเปลี่ยน fibrinogen ไปเป็ น fibrin
หมดฤทธิ์เร็ วมากมี half life 1-3 ชม. และมีฤทธิ์นาน 4-6
ชม. เมื่อหยุดยาเลือดจะแข็งตัวตามปกติในเวลาไม่นาน
ถ้าต้องการทาลายฤทธิ์โดยเร็ วทาได้โดยฉีด protamine
sulfate 50 mg, IV
การวางแผนการรักษาทางทันตกรรม
ซักประวัติโรคและปรึ กษาแพทย์เพื่อควบคุมระดับ heparin
ตรวจ PTT และ clotting time ก่อนทา
การทาศัลยกรรมช่องปากควรทากรณี เร่ งด่วนจริ ง
เมื่อต้องหยุด heparin หรื อให้ protamine sulfate แก้
ฤทธิ์ ไม่ต่ากว่า 6 ชม. จึงเริ่ มทาการถอนฟันหรื อผ่าตัดเล็กได้
เตรี ยมสารเร่ งการแข็งตัวของเลือด

เริ่ มใช้ Heparin ทันทีที่เลือดหยุด, ให้หลังถอนฟัน 6-12 ชม.

กดการสร้าง factor ที่ตบั ขัดขวาง γ-carboxylation ของ

ซักประวัติโรคและปรึ กษาแพทย์เพื่อหยุดยา
ตรวจ PT ก่อนทาผ่าตัด, ผ่าตัดเมื่อ PT ~ 1.5 -2 เท่า ,
INR ≤ 3.0
หรื อให้ prothrombin activity > 30 – 40%
อาจให้ Vit. K 10 mg IV, 6-8 ชม.
เตรี ยมสารเร่ งการแข็งตัวของเลือด
Antibiotics

หยุดยา ~ 3 วัน ก่อนผ่าตัด
หลังผ่าตัด เริ่ มยาได้ในตอนเย็น
Antibiotics ไม่ควรใช้ sulfonamides,
erythromycin, metronidazole, tetracycline
ใช้เตรี ยมสารเร่ งการแข็งตัวของเลือดเฉพาะที่
Aspirin
Antiplatelet aggregation
รบกวนการสร้าง factor II และ VII ในตับ
การวางแผนการรักษาทางทันตกรรม
ตรวจ bleeding time , PTT ดู coagulation phase
ถ้านานกว่าปกติ ปรึ กษาแพทย์เพื่อหยุดยา
หยุดยาไม่นอ้ ยกว่า 5 วัน
เย็บแผล
ใช้เตรี ยมสารเร่ งการแข็งตัวของเลือดเฉพาะที่
หลังผ่าตัด เริ่ มยาได้ในวันรุ่ งขึ้น หรื อเมื่อเลือดหยุดดีแล้ว
