2. Go to evgarebate.com and enter the promotion number 67953.
3. Fill out and print the registration form for this promotion.
4. Cut out and enclose the ORIGINAL UPC code from the product packaging. An
example of a UPC code can be found at the right of these instructions.
5. Enclose a copy of the sales receipt dated between 04/01/2016 and 04/30/2016
