     Originated out of a recommendation by the HIV QMI on June 6, 2012 to increase the efficiency of clinic flow for new patients Started project in August 2012 to eliminate 2 initial visit nd visit within 2 weeks of initial visit and to initiate HAART and/or OI prophylaxis on the Goal-75% of patients to complete initial enrollment/labs visit prior to 1 st office visit Benefits-Familiarity of clinic location, staff and clinic flow, initial rapport building, decrease wait time for the initial visit, lab results/HAART initiation and /or OI prophylaxis when warranted on initial visit and eliminate a 2 nd visit to start HAART/review of labs Barriers-lack and/or limited transportation, work schedules, inability to contact patient or too ill to keep scheduled appointment

     Used PDSA model for improvement Improvement in the time of initiation of HAART/OI prophylaxis of new patients Improvement in data collection and reporting Improvement in communication among the health care disciplines Extended lab hours and added transportation shuttle service to help eliminate barriers

     Will continue this project until August 2013 to reach goal Will continue to work with patients to improve adherence to initial enrollment/labs visit Staff, HIV QMI committee and administration had strong buy-in for project Will also monitor those newly diagnosed patients from August to present for retention in care Will continue to improve on eliminating barriers to keep initial enrollment/labs visit
