2 ND January WHAT HAPPENED?

The operation at the time of the incident was rigging down the top drive (TDS). The IP was on the manrider on the off driller's side. The retaining pin (TDS to torque tube) had been removed. The IP signalled the driller to lower the block. The guide beam became unexpectedly free, allowing the TDS assembly to rotate. The IP's right hand became crushed between the TDS carrier and a derrick beam.

RESULT

Two deep cuts in his hand and wrist.

16th January WHAT HAPPENED?

A 7 casing float collar was being cleaned/dried up on the rig floor using an air hose disconnected from the pipe spinner. The air hose ball valve was half open. After cleaning the collar the Derrick man (IP) who was holding the hose asked the floor man to close the air valve, but the floor man instead of closing, opened the valve to full, causing the hose to swing back & hit the IP's nose. •

RESULT

He suffered a fracture to his nasal bone •

22 nd January WHAT HAPPENED?

A crew was installing crash barrier posts using a small Pile Driving machine. The post had been driven to a depth of 1 meter when the Charge-hand noticed that it was slightly of vertical. He wanted to correct this, took a fist-hammer and forcefully started hitting the post. The hammer was in poor condition and the head detached from the handle at the second blow. •

RESULT

He suffered broke a finger •

20 th January WHAT HAPPENED?

4 1/2" tubing was being picked up to the V-door. Once the Tubing was at the V-door level, the elevator was latched into the tubing and the winch line removed and left at the rig floor. The Driller proceeded to lift the block and assistant driller (IP) was holding the tubing. When the block was picked up, the block caught the winch line and moved it toward the tubing. •

RESULT

He suffered right hand index finger. •

An adhoc ( BDHL) articulated vehicle transporting drilling salt between Muscat and Nimr, when 27km north of the town of Haima veered off the road and rolled over. •

1 st March WHAT HAPPENED?

Contractor were carrying out an offloading operation of 2 roll-over simulation cars from a truck. After the first car was offloaded, the IP went to pull the sling from under the second car. When he reached the edge of the trailer, he lost his balance and fell BACWARDS to the ground. •

RESULT

Head trauma, fractured right scapula and fracture on the left thumb. •

12 th February WHAT HAPPENED?

The Inspection Supervisor (IP) had returned from leave and was heading to Fahud to resume work in an ONTC bus. The bus entered the Wadi Adai R/A at high speed without observing the incoming Water Tanker, resulting in a collision. •

Hip fractures, multiple abrasions, friction burns and skin loss on left thigh.

During pipe connection in mouse hole, the Floorman (IP) held the break out tong pull line (that was already latched on to single pipe) with his right hand, while the kelly spinner was operating to make up the connection. As the pipe rotated clockwise in the mouse hole while the threads were being engaged, the tong rotated causing the line to jerk under tension. •

13 th March WHAT HAPPENED?

the IP was doing isolation of an electrical panel for the Beam Pump. He isolated the mains from a 415 V feeder by turning the isolator knob to “off” position, opened the feeder panel and removed three fuses. While attempting to clean the upstream connection of the isolator using a bristle brush, an electrical flashover occurred. The metallic part of the brush came in contact with the two phase terminals, resulting (IP).

RESULT

2nd degree burn to face & submandibullar area, right upper limb, right side of chest & abdomen & patchy areas at right lower limb. •

The Helper (IP) was standing on a second layer of 9.5/8” Casing trying to position one of the pipes that were loaded, with a crowbar. As he used force to set the pipe, the IP stepped back and slipped on grease that was on the pipe edge. He lost his balance, fell from the trailer onto the ground. •

The Weatherford UBD crew were moving temporary paperwork. A Field Operator (IP) and his colleague were carrying a 2-ft long Teclock pup joint (35 kg wt) to store it on the rack. As they were walking towards the rack, the colleague's glove slipped from the load, the joint dropped, subsequently hitting the IPs right foot. The IP sustained trauma on his right foot •

A Scaffolder (IP) was engaged in the erection of scaffolding that was to be used to inspect a heat exchanger on the station. The IP slipped and fell on the ground, from a height of about 4 meter.

14th May WHAT HAPPENED?

After picking up and running in hole 4.1/2 inch liner, stands of drill pipes were being run from the mast. The Derrickman (IP), was assisting the floorman (Derrickman reliever) to release the chain which was securing the drill pipes stacked on the monkey board. The IP partially sat near the stacked stands of drill collars and in the movement path of the Kelly hose. Whilst trying to release the chain, the Topdrive was moved downwards for running in hole and the Kelly hose rubbed the IPs helmet causing the helmet to turn, which then got caught with the downwards moving clamp of the Kelly hose squeezing the helmet onto the jaw. •

Sustained injury to his jaw.

An Assistant Driller (IP) and a derrick man were assembling the sucker rod BOP on the ground. While trying to re-align the extension flange type to the bottom of the assembly, the sucker rod assembly stripped off from the rod table thread connection, trapping the IPs right hand middle finger between the flange and the assembly.

A Mechanic (IP) was replacing the casing running tool (CRT) dies & packer cup from 9 5/8 inch to 7 inch. While unscrewing the packer cup screw, trapped pressure was released. This caused the casing segments part to be pushed upwards, between the packer cup & segments.

While working on the construction of a Steam Injection Manifold, the crew used hydraulic jacks mounted on timber to elevate approximately 80 m of 8" flowline, to allow surface preparation for welding of pipe shoes. While the Fitters Helper (IP) was engaged in the activity of positioning the last of 5 jacks, the flow line slipped, hitting him across both arms and his right thigh •

A 50 T crane was lifting a pressure equipment skid from the wash bay area to the wellhead equipment area. The crane operator was getting ready to position down the load to the ground when suddenly the load started to drop down in an uncontrolled manner. This caused the crane to topple on to its left side, hitting the roof of a workshop and the wall of the wellhead equipment area. • •

03 rd June WHAT HAPPENED?

While working on road asphalting project, borrow pit location, a Helper (IP) was trying to start a dewatering pump of the brackish tanker (pulley type starter) for pumping the water to a borrow pit. His right hand slipped while pulling the starter and his right middle finger got trapped in the rotating shaft of the pump.

RESULT

The IP sustained crush injury to his right middle finger tip with partial amputation and fracture.

17 th June WHAT HAPPENED?

A Floor man (IP) was working on the monkey board (an elevated work platform within the rig mast) as a relief Derrickman during the tripping of drill pipe out of hole.

Initial investigations suggest that the IP's fall arrestor line snagged on the empty travelling block as it was being lowered causing the IP, who was racking drill pipe, to fall on the monkey board. The fall arrestor line snapped under tension, struck the IP •

WHAT HAPPENED?

On 14th July 2010 at around 07:00 am, a team with two graders and one roller was directed by a supervisor to one of the Reed Bed sections to complete the leveling of the area. Material (sand) was offloaded by tippers to be spread in the area for compaction and leveling. The graders were working on the distribution of the material while the roller was idle. At around 10:30 am during a maneuvering operation one of the graders reversed onto the person who was counting the load trips near the piled material, crushing him.

While IP walking on the platform walkway looking up to get the spanner the IP failed to observe that one of the walkway gratings had been removed by the Instrument technician working on the level switch. The missing walk way grating is about 1.3m high from ground and no barricading was provided. The IP fell of approximately 1.3 meter, hitting the level switch and piping work beneath the gas lift skid.

A Truckoman driver (IP) was delivering directional drilling motors to Rig-82. He attempted to remove the secured belt from the motor but was unsuccessful. The motor was then suspended about 10cm using a forklift. The IP was removing the sling at which point the motor rolled off the fork, and landed on his right foot.

The Floorman (IP) was trying to repair a power tong that had a failure on the back-up jaws. Although the instructions from driller was to rig down the power tong and pick up the backup set, the IP at his own discretion decided to try to fix the failure. While attempting to do so; he inadvertently engaged the rotary (Upper jaws) while his left hand was placed inside the tong unit.

During rig move operations, the Floorman (IP) was in the process of removing a clamp on one of the mast cylinders in order to carry on with mast lowering operations. The IP stood on the edge of the carrier to remove the bolts of the clamp using small spanners. There was no handrail on the mast carrier where he stood and he was not wearing a safety harness (mast carrier height 2.6 m from the ground). He lost his balance and fell backwards onto the ground. suffering •

At around 14:00 hrs, while picking up 3-1/2" drill pipe and drifting the same with a 2" drift (2Kg weight), a floor man took a shortcut and threw the drift from the rig-floor (6m above the catwalk). The drift was to be used for drifting the subsequent drill pipe. The Derrickman (IP), who was standing near the catwalk, was hit by the drift on his helmet and upper back. •

After welding together two sections of three meter pipe mounted on a pipe stand, and allowing the weld to cool, the welder (IP) returned to check the alignment on the job. He altered his posture to look along the length of the pipe and took approximately three steps backward to examine the alignment. As he stepped backwards he tripped over a flange located on the ground ready for the following mornings fit - up and fell onto his right elbow.

19 th September WHAT HAPPENED?

Rig electrician was attempting to place a rubber mat under the 2 x 12 v batteries of engine #1. The chief electrician was standing nearby. A +ve terminal came in contact with lube oil filter creating a electrical short circuit which led to an explosion and fire. This resulted to severe burns to the electrician, who later died, minor burns to the chief electrician and damage to the engine.

• Battery hazard awareness (terminal leads fully covered).

• PTW system must to be applied for any job requiring electrical or mechanical isolation. Even for 12 v batteries.

5 th October WHAT HAPPENED?

During laying down of 8 '' bottom hole assembly, the drilling crew picked up the 6 ½'' Jar from the mouse hole by using a hydraulic winch operated by the Derrick man. Once the jar was out of the mouse hole, the Floorman (IP) pushed the jar with his left hand to the V-door while his right hand was holding the handrails. In the process of lowering the jar into V door, the Jar swung and hit the IP's right hand finger •

The operation at the time of the incident was removal of blown sand from the Well Pad Area of WSW-38. There was accumulated sand on the wellhead cellar which was manually removed and thereafter the crew decided to use the JCB and belt to remove the cellar grating frame (as it was too heavy to be manually lifted). While in the process of removing the frame, it swung, hitting the left leg of the Charge hand (IP), who was positioned between the JCB and the frame.

The IP is a 31-year old Roustabout working for MBPS a PDO contractor. While picking up a 6 ¼" Accelerator from the catwalk, when he placed his right hand in between the work string and the 6 ¼" accelerator.

24 th November WHAT HAPPENED?

he IP was involved in cable termination activities in the Auxiliary Room of the On Plot Control Building. Nearing morning tea-break, he went to look out of the window of the Emergency Exit door to see whether the tea pick-up was nearby. As he turned away from the door, he missed his step and put his right leg into a recess where a floor panel had been removed for a cabling activity.

RESULT

Fracture in the mid shaft of the left tibia and fibula.

21 st November WHAT HAPPENED?

While the Frac Assistant (IP) was approaching a wet ground (caused by guar gel spill during post frac operation), a water hose fell from the PCM gel processor to the spilled guar gel which splashed on the IP's face.

Eye epithelium abrasions with matted lashes & signs chemical burn on his left eye •

21 st November WHAT HAPPENED?

A Diesel Tanker was travelling from Fahud to Yibal. Approximately 15 km before Yibal, the driver (IP) lost control and the Tanker tilted towards the driver's side.

28 th November WHAT HAPPENED?

The IPs are a 30-year old Driver and a 26-year old . A Waste Skip Truck, with a skip mounted load, was on a return trip from Barik, going towards Qarn Alam. About 10 km from the Saih Rawl blacktop road, the driver lost control and the Truck rolled over. •

Multiple bruises on his left knee •

28 th November WHAT HAPPENED?

The IP is a 38-year old Painter. During the rig move, the IP was pulling cables near the PCR unit. IP slipped and his right knee hit against cable tray causing injury on his right knee. He continued working and only went to see medic later when pain and swelling increased. •

Fracture in his right knee. •

28 th November WHAT HAPPENED?

The IP is a 24-year old Floorman working for NDSC.

The 13.3/8’ coated casing was already run when the IP was making up surface cement line. Installing the chicksan was ongoing and one side of it was installed. While attempting make-up on the other end, IP's right hand got trapped between the chicksan and T-junction piece.

Crush injury to his ring & middle finger.

11 th December WHAT HAPPENED?

The Rigger (IP) was engaged in a pipe fit up task approximately 6 meters above ground. During alignment, the IP attempted to walk on adjacent pipe work to cross a gap of approximately 1.5 meters between two scaffold structures. He was wearing fall arrest equipment but had not secured himself. He slipped from the pipe and fell to the ground below landing on his lower back •

Fractured pelvic bones.

7 th December WHAT HAPPENED?

A 3 ½" Drill pipe was being run in hole for clean out. Once the Drill pipe single was at the V-door, a drift was dropped inside the pipe. The Drill pipe single was then picked up to the rig floor and the IP proceeded to open the drill pipe protector at rig floor to remove the drift from the pipe. Before opening the protector, the IP inserted his right hand middle finger through the protector hole to check if the drift was resting on the protector. At this time the stuck drift (3kgs) inside the pipe dropped down, trapping the IP's finger against the protector •

RESULT

Displaced fracture on the right hand middle finger (head of distal phalanx). •

05th December WHAT HAPPENED?

As the CTU supervisor, (IP), was watching the load being lifted during rigging up of the tower by a Crane, he moved sideways to communicate with the crane operator. He accidentally stepped into the location drain channel (around the location cemented pad), causing him to lose his balance. He fell down and hit his left knee against the edge of the drain channel •

RESULT

Hairline fracture on his left leg •

09 th December WHAT HAPPENED?

Whilst climbing down an unsecured ladder from the top of a scaffolding platform, the Assistant Operator (IP) lost his balance and fell from a height of 3 meters.

Sprained ankle.

18 th December WHAT HAPPENED?

The crew of Hoist 40 were engaged in fishing activities on a stuck completion. While attempting to fish the standing valve (SV), two floormen were using a 36" pipe wrench to turn the polished rod to the right, while they were doing this the driller picked up the string in order to fish the SV. As the rod rose up, the back torque was not controlled by the floormen, allowing the pipe wrench to spin around striking both of them in turn. The first floorman (IP/LTI) was struck on the face under his left eye.

RESULT

Open wound, facial and nasal fractures. The second floorman was struck on his left shoulder causing minor bruising and was classified as a First Aid Case •

A Technician (IP) was trying to remove debris from the filter of the blasting unit with the help of a metal scrapper near the water blasting area. He stretched his body forward and lost his balance, landing with his body weight on his left wrist.

One of the IP's suffered 2 nd degree burns on both hands & 1 st degree burns on the face. The High potential incident of 16 th December has been re-classified as LTI#73.
