8 years with the Damon system: Where am I now?
Who would not be allured by the buzz-words: “low-force”, “biologically
sensible”, “moving teeth with bone”, “reduced treatment time”, “more
patient comfort”, “no more RME, transpalatal arches and other
auxiliaries”, “ less extractions”, “face-driven treatment planning” and so
on?
The encouraging results of in-vitro studies and of treated clinical
patients made our mouth water and so we changed from our oldfashioned straight-wire appliances to the Damon System.
At first we were impressed how apparently well the initial leveling and
alignment and arch expansion proceeded. Indeed we decided to
perform less and less extractions and banished additional expansion or
anchorage devices. This was the time when “Damon or die” was our
motto.
After the first enthusiasm we encountered difficulties during the later
stages of treatment when rotations were still not perfectly corrected,
small spaces would not close and finishing was extremely laborious. A
considerable amount of brackets had to be re-bonded and high elastic
force level had to be applied for residual space closure and
achievement of ideal occlusion. Because of this dilemma we started to
utilize both bracket systems concomitantly - and to our surprise teeth
aligned and leveled equally well, arch expansion was equivalent and
overall treatment time was exactly the same.
After more than 1000 patients treated with the Damon System we
returned to our traditional straight-wire appliances and compared the
performance of the two systems. The results of our and other clinical
study and the critical evaluation of our patients’ records show that at
present the manufacturer’s claims regarding superior performance of
the Damon System compared to conventional bracket systems lack
clinical evidence apart from reduced chair time.

Confessions of a Damonist
I will present some personal reflections on why we choose the
orthodontic technique we use, and why we stay with it. In this context I
will touch on the importance of brackets and methods for the outcome
of the treatment. My experience of implementing the Damon technique
will be used to highlight both the difficulties and the inherent force in
exploring new boundaries. Statistics from my own office will be
compared to Damon paradigms. I will also point at both ups and downs
and try to explain why I am certain that I will stay with Damon, also
taking into account the financial aspect.

Extractions and control of expansion in the Damon System
To me the Damon system is not a non-extraction system, and so I will
present my way to handle extractions; Maximum and minimum
anchorage in different cases.
I will also show how to prevent unwanted expansion when you treat
with the Damon System.
