Application form and information brochure will be available from 10.12.2014 at all the private dental
colleges running the MDS course under the association against payment of Rs. 1500 as
examination/test fee. Information brochure can be downloaded from website free.
Application forms and OMR (Optical Mark Reader) sheet must be filled by using blue point pen. You
must read the instructions and fill up correctly and clearly as per directions given in the information
brochure. Once the application form and OMR sheet is filled up, No change should be made. There
should not be a mistake, cutting or over writing. No alteration will be permissible in the application at
any later stage.
All necessary documents are to be attached with the application form. The application form/ result /
counseling/ admission will be cancelled at any stage if any relevant document/ information has been
concealed or falsified.
Serial No. of application form / OMR sheet must be mentioned in all future correspondence.
Do not fold, staple or use pins on OMR sheet as the computer may not read the OMR sheet.
Candidate must affix latest passport size photograph on the space provided on application form and
OMR sheet and send two spare photographs along with the application form. The photograph should
be attested by the Principal/Dean of college last attended / or a class I Gazzetted Officer.
A candidate if employed, must have a No Objection Certificate signed by the employer for appearing
the entrance examination, failing which application may be rejected.
Application form must be accompanied with :
1.9.1. Attested photocopy of matriculation certificate.
1.9.2. Attested photocopy of 10 + 2 Pass certificate and mark sheet.
1.9.3. Attested photocopy of mark sheets of all professional examination of BDS.
1.9.4. Attested photocopy of Provisional/Permanent dental registration of DCI / State Dental Council.
1.9.5. Attested photocopy of compulsory rotatory internship certificate.
1.9.6. Attested photocopy of BDS passing certificate.
All original certificates will be produced by the candidate at the time of counseling/joining, any failure
or concealment/falsification of facts will lead to cancellation of candidate's claims to any seat or any
benefit accruing their after for appearing in examination.
Applicants should send their application forms well in advance by ordinary post so as to avoid rejection
on account of postal delay.

The association of Pvt. Dental & Medical colleges of UP may amend any order / instruction in
information brochure.
The number of seats as shown in information brochure may be amended as per permission of Govt. of
India and Dental Council of India.
All disputes pertaining to the UPCAT MDS -2015 shall fall with in jurisdiction of Distt. Lucknow.
In case of ambiguity in rules, interpretation by the Association of Dental & Medical Colleges of UP shall
be final.
The counseling and allotment of seat will be as per merit and choice of candidate.
The waiting list will be institution based & if any vacancy arises due to non-joining of the candidate
seats, it will be filled from waiting list by the concerned college under intimation of the Co-ordinator,
UPCAT MDS - 2015.

DURATION OF COURSE AND NUMBER OF SEATS AVAILABLE FOR
MDS IN VARIOUS COLLEGES
DURATION OF COURSE :
The period of training for obtaining MDS Degree shall be three completed years including the period of
examination.
No transfer of seats from one subject to the other and one college to other college shall be allowed after final
counseling.

IMPORTANT INSTRUCTIONS :
1.
A Candidate appearing for entrance examination for MDS will be eligible only if she / he :
1.1 has passed BDS examination from Recognised College / University in U.P. or any other state in
the Country.
1.2 has completed satisfactorily one year compulsory rotating internship from centers recognized by
the Dental Council of India for this purpose.
1.3 registered permanently with State / Dental Council of India at the time of admission.
1.4 possesses certificate of good conduct from the Head of the Institution last attended.

The Candidate will also submit in original the above mentioned certificates (i-iv) at the time of
counseling / joining without which admission to P.G. Course shall not be granted.

A student, who is already admitted to any course in any institution can only be eligible to apply for any
other course only after passing the first course. An undertaking to this effect will be obtained from the
candidate. Concealment of facts would invite disqualification.
PROCEDURE FOR FILLING UP APPLICATION FORM
The Application form & OMR Sheet attached in information brochure should be filled correctly in candidates
own handwriting and submitted to respective college latest by 31st January, 2015
The application Form must be accompanied by the following certificates
a)
Certificates of having passed BDS examination from institution recognized by DCI
b)
Four copies of the recent photographs (passport size) duly attested by the Director/Principal of the
College / Gazetted Officer or the employer in the case of those in employment.
c)
Internship completion certificate certifying the date on which the candidate has completed one
year's compulsory rotating internship.
d)
A Certificate of good conduct from the Head of the Institution last attended.
e)
Certificate of permanent Registration with State / Dental Council at the time of admission.

If any candidate fails to receive the admit card or losses it, must report to the center of examination and
produce a proof of his/her identity and on proper verification the candidate will be permitted to appear
in the examination with duplicate admit card.

The examination will be held on 8 February, 2015 at 3:00 PM to 5:00 PM at Lucknow & Delhi.
Admission in the P.G. Courses shall be as per DCI norms.
Syllabus will be as prescribed by the University for BDS course. The questions will be of BDS level,
covering complete BDS course. The question will cover the following subjects.

The examination will be conducted in English medium only.
In case of two or more candidates obtaining equal marks in the entrance examination, the inter-Se rank
will be decided in order of preference as under4.1

The examination will consist of one paper of 100 marks carrying multiple choice questions.

The candidates are required to carefully read the instruction sheet on the question booklet
before attempting. The method of marking responses will be clearly indicated on the
instruction sheet. No deviation other than the method prescribed will be considered.
WRITING OF PARTICULARS :
The candidate must fill up the required particulars including his / her Roll No. Centre of Examination,
Test Booklet Number and Signature on the OMR/ Answer sheet in the appropriate boxes, else he / she
will be awarded 'Zero Marks. He/She must NOT write his/her Roll No anywhere else on the answer
sheet.
MARKING OF ANSWER SHEET :
Full credit, i.e. half mark (1/2) will be given for each correct answer. There will be negative marking in
the entrance test. 1/4 mark will be deducted for every wrong answer. More than one choice given by
the candidate will be considered as wrong answer.
PROCEDURE TO BE FOLLOWED IN THE EXAMINATION HALL :
a.
Each candidate will be given a Sealed Test Booklet with an answer sheet placed inside it.
b. (i) Immediately on receipt of the Test Booklet as well as the answer sheet, the candidate will first
read the instructions on it and fill in the required particulars on cover page of the Test Booklet
and on the OMR answer sheet. Before starting the question paper, the candidates must check
that the test Booklet supplied to them is complete in all respects with regard to the number of
pages as well as the questions. Any discrepancy detected should be brought to the notice of the
invigilator immediately and the Booklet should be got changed. No complaint in this regard
will be entertained 30 minutes after the test is started.

The candidates will use blue ball point pen only while darkening boxes on the answer sheet.
Use of black pen or pencil is not allowed. Any violation of these instructions will lead to the
rejection of answer sheet straightway.
The test will start exactly at the time mentioned in the Information Brochure and an
announcement to this effect will be made by the invigilator. No candidate will be allowed entry
in the Examination Hall 15 minutes after commencement of the test.
During the examination time, the invigilator will check Admit-Card and compare photograph
of each candidate to satisfy himself about the genuineness of each candidate. The invigilator
will also put his signature on the place provided inthe Answer - Sheet as well as test booklet.
The candidates shall also be required to sign attendance sheet.
After completing the test and before handing over the Test Booklet and Answer Sheet, the
candidate should check again that he/she has followed the instructions printed in test -booklet,
that all the particulars required in the test booklet and the answer-sheet have been correctly
written. Ensure that the Roll No. is corectly written in the OMR/ answer sheet.
The OMR Answer Sheet should be handled by the candidate very carefully. It should not be
folded or mutilated in any manner, so that complete and accurate evaluation of the answer sheet is ensured.
To answer any question, the candidate will be required to darken the circle by using Blue Ball
Point Pen only corresponding to the answer, he / she thinks to be correct against the serial
number of the question as per example given under serial no. 9 below.
Sample Questions along with method of answer is given below :
Q.6 when a tuning fork vibrates with 1.9m or 1.05m long wire of a konometer, 5 beats per
second are produced in each case. What will be the frequency of the tuning fork?

The candidate will be required to write Roll No. and other particulars on the OMR / answer
sheet as shown below in the example :

The test -booklet and the OMR/Answer-sheet are to be handed over to the Supervisor /
invigilator before leaving the hall. The candidate who does not hand over the test -booklet
along with the OMR sheet to the Supervisor / Invigilator, his / her candidature will stand
cancelled besides facing action to be decided by the Association of Private Medical Dental
Colleges of U.P.
The candidates are required to furnish the Admit Card at the time of
(i) Entry into the Examination Hall & (ii) during the course of Examination for necessary
identification by the Supervisory Staff on duty. The Candidates are, therefore, advised to keep
the Admit Card intact.

USE OF UNFAIR MEANS :
If any candidate is found guilty of any breach of the conditions mentioned in the information brochure
or of using unfair means, his / her candidature shall be cancelled.
DECLARATION OF THE RESULT :
The result will be put on the website of the association : www.upcatdental.net
COUNSELING :
A central counseling will be held Online by a Committee comprising of Representative from all
colleges under the Chairmanship of Co-ordinator, UPCAT MDS-2015. The date will be intimated on
website of the association. Once a seat is accepted by a candidate will not be changed, unless he / she
has opted for waiting list in a particular subject in a particular college.
ADMISSION :
A candidate, who has been allotted seat in counseling, will report to the respective college's principal
along with all concern original documents and complete all the formalities for admission including
deposition of the prescribed fee by the date notified. Those who fail to do so, will loose a claim for
admission and the seat will be allotted to next candidate in waiting list.
Academic Session will start on 1st May 2015.
Any Dispute arising due to any reason subject to Lucknow Jurisdiction only.
Fee approximately Rs. 8.5 lacs (Eight Lacs Fifty Thousand) per year. College security, Hostel Charges, Hostel
Security, University Examination Fee are extra. Contact College of choice for details of all fee as the fee may
vary from college to college.
