8TH GRADE FAREWELL PARTY FRIDAY, JUNE 5TH, 2015 “SHOOTING STARS” All 8th graders are cordially invited to an unforgettable evening on Friday, June 5th, 2015. “ Shooting Stars “ is a celebration of the 8th grade class for three years of hard work and dedication. Each child is a “STAR” here at Van Hoosen, and this is our way of saying “good luck” as they embark on their next big adventure in life …High school! ADMISSION TICKET: $20 (non -refundable) **Tickets will NOT be sold at the door** HOW TO PURCHASE A TICKET: Enclose check (Van Hoosen PTSA) & permission/waiver, Attention 8th Grade Party on envelope. You can drop in drop box marked “8th Grade Party” in main office, or mail to Van Hoosen, 1339 N. Adams Rd, 48306 . If you have financial concerns, please contact Mr. Mooney directly to discuss. Van Hoosen Middle School Doors open at 6:00pm Ends promptly at 8:30 PM ***Casual dress, please follow Van Hoosen Dress Code*** WE NEED A PHOTO OF YOUR STUDENT, 2 YEARS OLD OR YOUNGER!! Please provide us with either a 3x5 or a 4x6 photo of your child. It needs to be an individual photo, of only your child. •Photo should be a copy, they will be returned as a mounted keepsake. •Label the back side of the photo with your student’s first and last name. •Please make sure the photo is in a sealed envelope, to the attention of 8 th Grade Party. •Have your student place the envelope in the box marked “Baby Photos” in the front office. A photographer will be taking pictures throughout the evening, and the pictures will be available for viewing on the Van Hoosen website. If you do not want your child's picture included, please email [email protected] by June 1st. Deadline to purchase tickets and send your pictures is March 27th. Adams Road, 48306. No ticket will be issued without the completed Permission and Waiver. Your cancelled check will be your receipt. Students will receive their actual ticket when they register at the door, the day of the party. STUDENT PERMISSION & WAIVER I hereby give my permission for my son/daughter___________________________ to attend the Van Hoosen 8th Grade Farewell Party on June 5th, 2015 from 6:00 -8:30pm. I understand that the games and athletic devices that will be available may pose certain risks To the participant. I hereby assume such risks for the aforementioned student and waive and hold Rochester Community Schools, the 8th Grade Party Committee and volunteers, and the Company and it’s employees and operators of such said games and devices harmless from liability relating to the use of said games and devices. I also understand that my child must remain at the party until the doors re-open at 8:30.
