3-Step Medication Reconciliation Process
Verify
• Determine a complete list of all
medications the patient is currently
taking
• Inquire about OTC medications,
vitamins, herbal supplements &
culturally-based remedies
• Obtaining a complete medication list
may be complicated by the patient’s
cognitive impairment, polypharmacy
and/or financial difficulties
Clarify
• Clarify medication names, doses
and frequencies
• Identify drug-drug interactions and
therapeutic duplications
Reconcile
• If no drug-drug interactions or therapeutic
duplications are identified, enter the
completed medication list on HCFA form
485
• Communicate any drug-drug interactions
and therapeutic duplications to the
physician
• Obtain revised physician orders and
document revised medication list
