Applying for an Insurance Waiver

Submit Waiver Application form and proof
of current insurance plan to CIE.

Last day to submit a waiver is:
June 24, 2013.

If waiver is not submitted before the
deadline or is not approved, you will be
held responsible for the full cost of the
insurance.

 Your insurance card has your Student ID
number, name, and other valuable
information on it.
 Keep this card with you at all times!
 Show your card upon arrival at a doctor’s
clinic or hospital.
 Update your correct address and Madison
College email at [email protected]
or a delay in coverage payment may occur.
How do I pay for a doctor visit?
 Many claims (bills) will be paid directly between
the insurance company and the doctor if the
student uses a network doctor
 Sometimes the student may have to pay first and
then get reimbursed
 Not all services are provided by the doctor
 Lab work (blood tests, urine tests, etc) is often sent
to a lab and will be a separate charge
 Prescriptions are a separate charge

Finding a Doctor/Clinic/Pharmacy:

Pharmacy” to search for an approved doctor close to
you.

Choose the GWH-CIGNA Provider network and
search by “Specialty” or Medical Condition” to find a
provider. “Select a Network” will default to PPO,
which is best. Under “Provider” type we recommend
Hospitals/Facilities. Under “Specialty” types, we
recommend selecting “Medical Clinic, Urgent Care
Center, or General Hospital” depending on need.
