Clinical & Systems Transformation Overview Date Name and Title of Presenter What is Clinical & Systems Transformation (CST)? CST is a project to improve patient care by making sure that our practices are evidence-based and consistent across clinical areas, and we are supported by smarter technology including an electronic health record. This will be a huge change in the way we deliver patient care, so dedicated education and support will be provided. 2 A joint initiative of VCH, PHSA, and PHC Why Are We Doing This?
