Clinical & Systems Transformation
Overview
Date
Name and Title of Presenter
What is Clinical & Systems Transformation (CST)?
CST is a project to improve patient care
by making sure that our practices are
evidence-based and consistent across
clinical areas, and we are supported by
smarter technology including an
electronic health record.
This will be a huge change in
the way we deliver patient
care, so dedicated education
and support will be provided.
2
A joint initiative of VCH, PHSA, and PHC
Why Are We Doing This?
Too many patients suffer from adverse events
Too much variation and too many workarounds
in patient care
“Accurate
information is
critical to providing
good quality and
safe patient care.”
Too much time spent chasing paper charts
and completing unnecessary paperwork
Too many duplicate tests
Too much time between an order being written
and the patient receiving therapy
Too many old, separate clinical information systems
