What is a case manager (CM)?

• a specialist, part of the multidisciplinary team, who
• coordinates the team that works with the child,
• develops and maintains the partnership with the family,
• develops and maintains the partnership with other
organizations and institutions involved in social and
educational inclusion.
How does one become a CM?

• any member of the team may be designated the CM
• the department coordinator names the CM amongst the
team members that work with a particular child

• plans the services schedule, based on the assessment results
provided by the psychologist and the medical doctor
• communicates the plan to the family and makes the necessary
adjustments
• plans and attends the team meetings with the parents, in one
month time after the child enters the services, so that the
therapeutic goals are set
• intermediates the meetings between the counselor and the
family
What does a CM do?

• calls a team meeting any time there’s something to discuss
• intermediates the partnership with the teacher(s), plans and
attends the meetings
• makes the appointments to the psychologist and medical
doctor once a year, for re -assessment
• plans and attends the team meetings with the parents, in one
month time after the child enters the services, so that the
therapeutic goals are set
What does a CM do?

• attends the weekly department meetings
• collaborates with the department coordinator and the team
• communicates to the department coordinator and to the
child’s team any change, news or update regarding the child
and his/her family
• operates changes regarding the schedule or therapeutic set
(under the consent of the department coordinator)

• coordinates the elaboration and development of the
individualized educational plan
• maintains the professional partnership with the Intern
Evaluation Commission and the County Complex Expertise
Commission
• prepares and organizes the documentation comprised in the
child’s file
