Acupuncture Treatment of Diseases Respiratory 3: Asthma 哮喘 Definition: 1. Characterized by repeated attacks of paroxysmal SOB, wheezing in the throat despite having the mouth and shoulders raised. 2. “Xiao” is characterized by wheezing in the throat. 3. “Chuan” is difficulty in breathing even breathing with the mouth open and shoulder raised. 4. Their causes and pathogenesis are similar and often occur simultaneously. 5. Common in cold season or drastic climate changes. 6. In western medicine, it is referred to as bronchial asthma, chronic asthmatic bronchitis, pneumonia and emphysema. Causes and Pathogenesis : 1. Basic cause is internal retention of phlegm fluid. • Can be partially blamed on food issues. • Leads to a dysfunction of the spleen. 2. In children, the attacks are usually induced by seasonal contraction of external evils. 3. In adults, usually induced by prolong cough, emotional disturbance and over exertion. 4. In acute stage, usually manifest as excessive pattern due to blockage of Qi and accumulation of phlegm. 5. In Intermission stage, manifest as deficiency pattern due to repeat attacks and impaired functions of lung and kidney. Clinical Manifestations: 1. Excess patterns Chief symptoms: i. ii. iii. Harsh deep and prolong breathing Preference for exhaling Forceful pulse in acute attack. Clinical Manifestations: 1. Excess patterns A. Wind-cold invasion type i. ii. iii. iv. v. vi. vii. viii. Coughing and panting. White watery sputum. Headache. Aversion to cold. No sweating. No thirst. Light coloured tongue with thin white coating. Floating, tight pulse. Clinical Manifestations: B. Phlegm-heat obstructing lung type i. ii. iii. iv. v. vi. vii. viii. ix. x. Coughing. Difficulty in expectorating sputum Oppression in the chest. Fever. Thirst. Poor appetite. Constipation. Red tongue. Yellow greasy coating Slippery, rapid pulse. Clinical Manifestations: 2. Deficiency patterns Chief symptoms: i. ii. iii. iv. Asthma with a low sound. Dyspnea. Weak constitution. Forcefulness and feeble pulse in general. Clinical Manifestations: 2. Deficiency patterns A. Deficiency of lung qi type i. ii. iii. iv. v. vi. vii. viii. Dyspnea with shortness of breath. Aggravated by exertion. Sputum wheezing in the throat. Lassitude. Watery sputum. Sweating on exertion. Light coloured tongue with thin white coating. Thready, rapid pulse. Clinical Manifestations: 2. Deficiency patterns B. Deficiency of kidney qi type i. ii. iii. iv. v. vi. Short and abrupt breathing. Increased exhalation and decreased inhalation. Asthma worsen with exertion. Cold limbs after perspiration. Pale tongue with thin white coating. Deep, thready pulse. Treatment : Excessive pattern • “Xuan” lung qi, disperse evils and dissipate phlegm Principle channels: 1. LU channel. 2. Conception Vessel. 3. BL channel. 4. EX-O acupoint Principle acupoints: 1. LU 7 Lieque 列缺. 2. LU 5 Chize 尺泽. 3. CV17 Tanzhong 檀中. 4. BL 13 Feishu 肺俞. 5. EX-B1 Dingchuan 定喘. Prescription explanation (Principle acupoint): 1. LU 7 Lieque 列缺: • Luo-Connecting point of the LU channel, chosen to “Xuan” the lung qi and dispel the evil qi. 2. LU 5 Chize 尺泽: • He-Sea acupoint of the LU channel where qi of the channel congregates, chosen to descend the averse rising of the lung qi. 3. CV17 Tanzhong 檀中. • Channel acupoint of the CV channel and Qi fluential acupoint , chosen to release the distress chest and regulates qi dynamics. 4. BL 13 Feishu 肺俞 . • Channel acupoint of the BL channel and Back-Shu acupoint of the lung, chosen to regulate the function of the lung and allow the lung to dissipate the phlegm. 5. EX-B1 Dingchuan 定喘. • Extra ordinary acupoint, effective point for asthma. Principle of treatment Excessive pattern: Wind-cold invasion type of asthma): • Expel wind and disperse cold evils. Principle acupoints: as stated Supplementary acupoints: 1. BL 12 Fengmen 风门. Principle of treatment Excessive pattern (Phlegm-heat type of cough): • Dissipate phlegm and disperse heat evils. Principle acupoints: as stated Supplementary acupoints: 1. ST 40 Fenglong 丰隆. 2. GV 14 Dazhui 大椎. 3. LI 11 Quchi 曲池. Treatment : Deficiency pattern • Tonify lung qi, moisten lung yin, enhance reception of kidney qi, regulate spleen and stomach Principle channels: 1. LU channel. 2. Conception Vessel. 3. BL channel. 4. KI channel. 5. ST channel. 6. EX-O acupoint Principle acupoints: 1. BL 13 Feishu 肺俞. 2. BL43 Gaohuang 膏肓. 3. BL23 Shenshu 肾俞. 4. EX-B1 Dingchuan 定喘. 5. LU 9 Taiyuan 太渊. 6. BL 13 Feishu 肺俞 . • Channel acupoint of the BL channel and Back-Shu acupoint of the lung, chosen to regulate the function of the lung and allow the lung to dissipate the phlegm. 2. BL 43 Gaohuang 膏肓. • Channel acupoint of the BL channel, chosen to tonify lung qi and moisten lung yin. 3. BL 23 Shenshu 肾俞. • Channel acupoint of the BL channel, chosen to enhance the reception of kidney qi . 4. EX-B1 Dingchuan 定喘. • Extra ordinary acupoint, effective point for asthma. 5. LU 9 Taiyuan 太渊.KI 3 Taixi 太溪. • Shu-Stream and Yuan-Source acupoint of the lung and kidney channel. Chosen to tonify the qi of the two channel. 6. ST 36 Zusanli 足三里 • He-Sea and Lower He-Sea acupoint of the ST channel, chosen to nourish the stomach and the spleen. Principle of treatment Deficiency pattern: Deficiency of lung qi type of asthma): • Tonify lung qi . Principle acupoints: as stated Supplementary acupoints: 1. LU 5 Chize 尺泽 . 2. CV 6 Qihai 气海. Principle of treatment Deficiency pattern: Deficiency of kidney qi type of asthma): • Tonify kidney qi and constitution. Principle acupoints: as stated Supplementary acupoints: 1. KI 10 Yingu 阴谷 . 2. CV 4 Guanyuan 关元. TCM medical advises: 1. Keep warm during time of season change. 2. Avoid exposure to cold and wind. 3. Good diet habit avoiding raw, cold and greasy, pungent food. 4.
