• Pay for qualified medical expenses (you decide when to use)

• You can contribute up to $3,100 employee only $6,250 family (+$1,000 if age 55+)
• Investment options for balances above $1,000 at UMB Bank include:
— Money Market Sweep Account
— HSA Brokerage Account
• Debit card – for tax-free withdrawals of eligible healthcare expenses
• Limited FSA – if participate in both HSA and HCFSA your FSA can be used ONLY for dental and vision
expenses
• No risk of losing the funds
— Funds roll forward each year (no use it or lose it)
— Take your HSA account with you (drop the plan, still have access to the funds for medical expenses)

Spousal Surcharge
Additional amount you pay for medical coverage if you enroll your spouse /domestic partner
in a First Data medical plan and they are eligible for coverage through their employer.
The surcharge does not apply if:
• You do not enroll your spouse/domestic partner in medical coverage
• Your spouse/domestic partner is not employed
• Your spouse/domestic partner is employed, but is not eligible for medical coverage
through their employer
• Your spouse/domestic partner is eligible for, or enrolled in, Medicare/Medicaid
• Your spouse/domestic partner is also a First Data employee
The additional surcharge amount is $900 per year deducted on a per-pay-period basis:
—$37.50 semi monthly pay period
—34.62 bi-weekly pay period
If during enrollment you fail to certify their status for medical coverage, deductions will
automatically begin with your first paycheck and will not be refunded.

$250 Tobacco-Free Incentive
• If you enroll in a First Data medical plan, and have not used tobacco in any form for a
year or more, a $250 annual incentive is available to you. Just select “I am Tobacco
Free” on the Wellness Incentive page on YBR.
$200 Health Screening Incentive
• Complete the onsite health screening in first quarter and receive an additional $200
annual incentive with credits beginning in May.
• Since the onsite clinics are only available during a select time each year, you will
automatically be credited with this incentive for 2012. Any future health screenings
will require your participation to earn the incentive.

2. You will be automatically enrolled after 30 days
• If you do not elect otherwise, you will be automatically enrolled with
a 3% salary deferral
• Your contribution rate will be escalated automatically by 1% each
year, up to 10% of your eligible pay or the IRS annual savings limit
• Your contributions will be invested in the Vanguard Target
Retirement Fund that most closely matches your target retirement
year, based on an assumed retirement age of 65
You can take action to change your contribution rate, diversify
your investments, or opt out of the ISP at any time
