SERVICES
Prescriptions
Will refill and continue to prescribe Appropriate
medications for all students while they are members
of the UMB community
Controlled substances including narcotics and
stimulants (Adderall, Ritalin, etc) require Detailed
previous medical records from you primary care
provider and Appropriate proof of testing.
Dispensation of this medications is up to the
Discretion of the student health provider.

Student Health will arrange for an exposed
student to be seen A.S.A.P.
PEP as indicated.
Follow up as indicated.
Students on rotations should contact that
facilities Exposure Hotline or Employee Health
then notify our UMB Student Health / UMIC.

Can see your own doctor – but pay co-pays
All vaccines & TB Skin tests will be billed to the
students insurance.

Student Health Insurance
Unlimited “routine” health care for all students.
• Colds, injuries, infections, pain
• Contraceptive issues
Non-routine care billed to insurance
• Minor surgery, colposcopy, IUD’s, etc.
Labs, X-rays, cultures – billed to insurance
Prescriptions – billed to insurance
***Insurance will cover 80% of medical billing***
Outside Insurance
Unlimited “routine” health care for all students.

IMMUNIZATION & MEDICAL
HISTORY FORM
Complete the front of the Medical History Form -You can do this.
Have your HealthCare Provider complete the immunization
portion or submit signed documentation.
All Dental Students are required to have a Hepatitis B Surface
Antibody test after having had all 3 doses of the Hepatitis B
vaccine.
Students can complete their immunization process with the
Student Health/UMIC clinic.
Students will not be allowed to register for their second semester
if the immunization portion is not in compliance.

Services /Forms and Policies)
