I, the undersigned, having filed a petition for the degrees o f Freemasonry in your lodge, do hereby
declare.

At the time I became a member of such lodge, I was unaware of the character of the lodge; was
unaware that the lodge was not recognized by or in amity with th e Conference of Grand Master of

clandestine.
I mistakenly believed that I would be a Master Mason with full rights and benefits to travel and visit
other brother Masons; and to affiliate, if I so desired, with any other Masonic Lodge in North
America.
As a Consequence of my ignorance, I did unwittingly connect myself with such lodge and did
receive the degrees.
Upon expressing my interest in affiliating with your lodge, I was made aware of my ignorance.
Having been brought to light, I do now declare, of my own free will and accord, that I renounce all
obligation, allegiance and membership in the aforesaid lodge and association; and vow and
declare that I will not in any manner, directly or indirectly, have or hold any intercourse or affiliation
with the aforesaid lodge and association.

A plural member is a Brother who holds a
Primary membership in another lodge in
Wisconsin or another Jurisdiction. If his
Primary lodge is in Wisconsin that is where
he pays his Per-Capitia Tax. He is
responsible for Per-Capitia Tax only once in
a Wisconsin Lodge. If his Primary lodge is
in a Foreign Jurisdiction he can maintain
his Primary membership in that
Jurisdiction and hold a Plural in Wisconsin,
however, in this case his first Plural
membership in Wisconsin is subject to PerCaptita Tax. Use Form 4b Rev.

Procedure
•
The procedure for Plural and Transfers is almost identical. Just make sure
you are using the proper form as noted above. The procedure is as follows.
•
The Brother who wishes to Transfer or Plural needs to present a current
Dues Card. He needs to fill out Form 4a Rev 1.1~8/2010 or 4b Rev.
1.2~8/2010.
•
The petition needs to be Received at a Stated Communication and voted on at a
subsequent Stated Communication. The Lodge Secretary needs to make sure the
entire form is completed, and if from out-of-state that the out-of-state section is also
completed. Form 4a Rev. 1.1~8/2010 and 4b Rev. 1.2~8/2010 are to be emailed
to [email protected] or mailed to the Grand Lodge office for processing.
The office will handle the balance of the transaction.Do not enter this into MORI Jan will handle that when all the information is received. We will also see that the
other lodges are notified. Do not correspond directly with out-of-state lodges or

Requests from the office
• Please make sure that all check you receive from the
GL are cashed in a timely manner. (after 6 months
they are void, reissues will have a fee applied)
• Please pay lodge invoices promptly, all of us pay for
extra invoicing and the labor involved.
• Out-of-state transfers should not be started after
November 1st and in-state transfers should not be
started after November 15th. If not completed by
December 31st the transferring member will still owe
dues to his current lodge to remain in good standing.

Checklist for Preparing 2012 Dues Statements
Preparing to run dues statements in MORI is quite easy, however, this is the first year for some of you
and for the rest you can use this as a review. Let's prepare a checklist to make sure all is in order.
Please make sure you follow the order of the list.
1. Lodge > Member Types: Verify dues amounts and make sure they properly reflect your current
dues and Grand Lodge PerCapitia. (PerCapitia is $37)
2. Financial > Initialize Dues: Use this to verify thay all members have a member type assigned to
them, if one is missing select the correct one from the drop-down menu.
3. Financial > List Paid / Unpaid: Run the report to verify any amounts still due the lodge. MAKE
ALL CORRECTIONS NOW BEFORE GOING TO STEP 4.
4. Financial > Edit Arrears / Dues: This screen allows you to edit problems from Item #2.
5. Financial > Setup Dues: Enter 2012 in the box and click 'Setup'. IMPORTANT - this will roll your
dues year and sequence your arrears. Make sure your information is correct before doing this
step!
6. Lodge > Dues Notices: Review and verify the information is correct. The colored areas are user
definable.
7. Lodge > Dues Notices: Select Print then decide how you want to print your notices; 3/page or
on top or on the bottom
Please remember that after November 15th you should not be making any transfers in or out
of lodge. Dues Cards have been mailed to the Lodge Secretaries.
