A PIA is a fact-finding exercise and a
chance to relate and record pieces of
information that collectively form a
picture of the event and how personnel
responded from both a command and
operational standpoint.
A tool to assess our effectiveness
Post Incident Analysis (PIA)
Identify positive outcomes that reflect proper
attention to procedures, good decisionmaking and leadership
Provide personnel with a clear indication of
the impact their actions had on the outcome
Identify any safety issues or hazards that can
be prevented from reoccurring at future
incidents
Help identify any additional or remedial
training for personnel
Identify any policies and or procedures that
can be refined for future performance

Describe the Actions Taken
A42’s initial actions of providing a short
report, spotting the hydrant and making
the supply for E42 was critical to the
success of the incident
Quick and effective knockdown of the fire
Good division of labor, assignments
quickly and clearly communicated
THINGS THAT WENT WELL
Use salvage tarps before pulling ceiling
Higher awareness of salvage during fires
Reduce unnecessary damage to structure
Turn off the main breaker and leave
individual circuits as you find them
WHAT WE CAN DO BETTER
All personnel need
to use proper PPE
when working at
fire scenes

None noted at this time
FOLLOW UP ITEMS
Room of origin was the
master bedroom on the
main floor. A failure of a
multi-plug appliance was
found underneath bed.
Device failure led to the
ignition of combustible
materials by bed.
