What is our approach?
• NPN Comprehensive Asthma Management Program
utilizes the implementation of a multidimensional
approach incorporating evidence based practices from
Boston Children’s Hospital Community Asthma Initiative and
The Children’s Hospital of Philadelphia Community Asthma
Prevention Program. NPN’s program will consist of peer to
peer support, in school time asthma education, and
home visitation based on 3 levels of intervention.
 It includes nurse case management and care
coordination combined with home visits
 Goal- educate families about asthma, assess the home
for asthma triggers, and provide materials and services
to improve the home environment, such as
encasements for bedding, HEPA vacuums and pest control.
